Abstract
Torakalni bolni sindrom označava skup simptoma kao što su bol, zatezanje, probadanje, osjećaj napetosti prilikom dodira ili trnjenje u području torakalne kralježnice s mogućim širenjem bolova u prsni koš. Pacijente ovakvi bolovi nerijetko uplaše zbog sličnosti simptoma sa akutnim infarktom miokarda, pa je pritom važno iskustvo liječnika kako bi se brzo isključilo ovo po život opasno stanje. Uzroci torakalnog bolnog sindroma su brojni, a najčešćim se smatra mišićno-koštana bol. Pacijenti se prezentiraju sa bolovima u torakalnom području koji ometaju njihove svakodnevne aktivnosti. Najčešće su kratkotrajni, ali se ponavljaju u više navrata tijekom života. Dijagnoza se oslanja na klinički pregled koji se nadopunjava sa slikovnim pretragama i to najčešće kompjutoriziranom tomografijom (CT), magnetskom rezonancijom (MR) i elektromioneurografijom (EMNG). U prevenciji ovog sindroma važno je izbjegavanje čimbenika rizika kao što su dugotrajno stajanje i sjedenje, prekomjerna tjelesna težina te nedovoljna tjelesna aktivnost. Glavni cilj liječenja je ublažavanje bolova i povećanje kvalitete života oboljelih. To se postiže primjenom analgetika, edukacijom o izbjegavanju provocirajućih faktora i fizikalnom terapijom. Medicinska gimnastika je najčešće primjenjivana metoda kojom se vježba ekstenzorna muskulatura kralježnice, povećava fleksibilnost kralježnice i pravilno držanje tijela. Ostale fizikalne metode koje se koriste su elektroterapija, terapijski ultrazvuk, elektromagnetska terapija i laser te lokalna primjena topline i hladnoće.
Abstract (english)
Thoracic pain syndrome includes symptoms such as pain, straining, sticking, tenderness to palpation or numbness in the thoracic spine with possible radiation of the pain in the chest. Patients who present with these symptoms are often scared, because symptoms are very similar to acute myocardial infarction, so it is important that doctors have a lot of experience to exclude this life-threatening condition. There are several causes of thoracic pain syndrome, and the most frequent is musculoskeletal chest pain. Thoracic pain interferes patient’s daily activities. Symptoms are usually short-term, but are often repeated during their life. Diagnosis relies on physical examination which is often supplemented with computerized tomography scans (CT), magnetic resonance imaging (MRI) and electromyoneurography (EMNG). To prevent this syndrome, patients have to avoid risk factors such as prolonged standing and sitting, excessive body weight and lack of physical activity. The main goal of treatment is to reduce pain and increase the quality of life. Treatment is based on analgesics, education about avoiding provoking factors and physical therapy. Medical gymnastics is the most commonly used method for extensional spine muscles which increases the flexibility of the spine and the proper posture of the body. Other physical methods which are used are electrotherapy, therapeutic ultrasound, electromagnetic therapy, laser ald local application of the heat and cold.
