Abstract
Zadnjih godina sve se više pažnje posvećuje ulozi crijevne mikrobiote u patofiziologiji širokog spektra bolesti. Većinom se proučava poveznica s kroničnim bolestima kao što su dijabetes 2, ateroskleroza, hipertenzija i kronična bubrežna bolest. Bakterije u probavnom sustavu nisu zajednica mikroorganizama koja samo pasivno nastanjuje naša crijeva, već predstavljaju vrlo aktivan metabolički sustav koji utječe na brojne funkcije domaćina. Svi ljudi dijele određene sličnosti u sastavu mikrobiote, no različiti genetski i okolišni čimbenici doprinose individualnim razlikama te stoga možemo gledati na mikrobiom kao na „crijevni otisak prsta“. Zbog sve učestalijeg zapadnjačkog stila života mijenja se sastav bakterija a tako se mijenjaju i njihovi produkti. Upravo ćemo u ovom radu objasniti vezu između mikrobiote i bubrežne funkcije te također obrazložiti učinak različitih proizvoda bakterijskog metabolizma na progresiju kronične bubrežne bolesti. Kod kroničnih bubrežnih bolesnika često je prisutna disbioza mikrobiote a osobito oni sa završnim stupnjem bubrežne bolesti imaju potencijalno veći rizik od izloženosti visokim koncentracijama ovih spojeva. Prehrana je jedan od glavnih faktora koji utječe na status crijevne mikrobiote, gdje ona bogata vlaknima i hranom biljnog podrijetla doprinosi prevladavanju simbiotskih vrsti te stvaranju potencijalno nefroprotektivnih spojeva. Opća je praksa u liječenju kroničnih bubrežnih bolesnika da se provodi restriktivna dijeta zbog straha od hiperkalemije povezane s povrćem i voćem i hiperfosfatemije vezane uz mliječne proizvode. Prehrani siromašnoj u tim namirnicama također nedostaju vlakna, polifenoli i vitamini koji dokazano pospješuju ravnotežu u crijevima. Iako postoje objektivni razlozi za takav strog pristup liječenju, novija saznanja govore u prilog korištenju prehrane biljnog podrijetla u tih bolesnika sa svrhom ispravljanja disbioze mikrobiote, smanjenja stvaranja uremičnih toksina te postizanja potencijalnog usporenja progresije kronične bubrežne bolesti.
Abstract (english)
In recent years more time is devoted for the exploration of microbiotas role in pathophisiology of different disease. The connection with chronic diseases, such as type 2 diabetes, atherosclerosis, hypertension and chronic kidney disease, is mainly analyzed. The bacteria in the gut are not just a microorganism community passively inhabiting our intestine, instead they represent a very active metabolic system affecting many of the hosts functions. All people share certain microbiota composition similarities, but various genetic and environmental factors contribute to individual differences and therefore we can view our microbiome as a „gut fingerprint“. Because of more frequent westernised diet occurrence, bacterial composition changes and so do their metabolites. In this paper we are going to cover the connection between microbiota and kidney function, as well as the effect of diverse bacterial metabolites on chronic kidney disease progression. Patients with chronic kidney disease often suffer from microbiota dysbiosis and those with end stage renal disease are at a potentially increased risk of high uremic toxin exposure. Diet is one of the crucial factors influencing intestinal microbiota status, the one containing fiber and plant based food beeing the most beneficial for simbiotic species and for production of nephroprotective compounds. General practice in chronic kidney disease treatment involves restrictive diet measures because of the fear of vegetable and fruit induced hyperkalemia and dairy related hyperphosphatemia. Diet deficient in these foods also lacks fiber, polyphenols and vitamins all of which facilitate intestinal balance. Even though there are objective reasons for such strict treatment approach, new findings suggest that plant based diet could be used in these patients for the correction of disbiosis, lowering of uremic toxin levels and potentially slowing down chronic kidney disease progression.
