Abstract
Dvojezičnost je široko rasprostranjena pojava u svijetu koja podrazumijeva upotrebu dvaju ili više jezika. Zbog zabilježenog porasta dvojezičnosti očekivan je porast dvojezičnih govornika u logopedskoj terapiji. Afazija kod dvojezičnih govornika zanimljiva je tema istraživačima i izazov u kliničkoj praksi. Dvojezičnost nije zbroj dviju jednojezičnosti te zbog toga dvojezični govornici s afazijom zahtijevaju nešto drugačiji pristup u procjeni i terapiji. Cilj diplomskog rada je dati prikaz procjene afazije kod dvojezičnih govornika vodeći se kompleksnošću koju nameće dvojezičnost. Dvojezično iskustvo utječe na funkciju i strukturu središnjeg živčanog sustava. Iskustva se razlikuju i zbog toga je pristup svakoj dvojezičnoj osobi s afazijom individualan. Procjena dominantnosti jezika prvi je korak u procjeni dvojezičnog govornika s afazijom. Važna je zbog točnije procjene i razumijevanja trenutnog jezičnog statusa, učinkovitog planiranja terapije i očekivanja ishoda. Najpoznatiji sveobuhvatni test za procjenu trenutnog jezičnog statusa dvojezičnih govornika s afazijom je Bilingual Aphasia Test (Paradis, 1987), dostupan i u hrvatskoj verziji koja nije jezično i kulturološki adaptirana. U procjeni dvojezičnih govornika potrebno je usmjeriti pažnju i na „dodatne“ simptome, poput teškoća u kontroli i odabiru jezika što rezultira patološkim prekopčavanjem koda i paradoksalnim prevođenjem. Svrha rada je osvijestiti dvojezičnost u logopedskoj procjeni i terapiji te pružiti smjernice logopedima za prvi susret s dvojezičnim govornikom s afazijom.
Abstract (english)
Bilingualism is a widespread phenomenon that involves the use of two or more languages. An increased number of bilingual clients in speech therapy is expected due to bilingualism being more widespread. Bilingual aphasia is an interesting and common research topic and a challenge in Croatian clinical practice. Bilingualism is not the sum of two complete or incomplete monolinguals. Therefore, bilingual speakers with aphasia need a slightly different approach in assessment and therapy. The aim of this paper is to present the clinical assessment of bilingual aphasia. Bilingual experience affects the function and structure of the central nervous system. Experiences vary and therefore each approach to bilingual aphasia is individual. The first step in assessing a bilingual aphasia is evaluating the language dominance. It helps to understand the client's current language status and leads to more accurate assessment, effective therapy planning and outcome expectations assumptions. The most well-known comprehensive test for assessing bilingual aphasia is the Bilingual Aphasia Test (Paradis, 1987). The Croatian version of the test is available but it is not linguistically and culturally adapted. „Additional“ symptoms of aphasia such as pathological code switching and paradoxical translation can be present in bilingual aphasia and therefore evaluated in the assessment. The purpose of this paper is to raise awareness of bilingualism in speech assessment and therapy by providing guidance for the therapist's first encounter with bilingual aphasia.
