Abstract Prema DSM V podjeli pedofilija, točnije, pedofilični poremećaj definira se kao, opetovane, snažne seksualno uzbuđujuće fantazije, seksualni porivi ili ponašanja koja uključuju seksualnu aktivnost s djetetom pretpubertetske dobi (općenito dobi 13 godina i mlađom) te su prisutni najmanje 6 mjeseci. Osim toga, izazivaju značajnu patnju i interpersonalne teškoće osobi koja pati od navedenog poremećaja te imaju određene specifičnosti kao što su isključivost, spol djeteta i ograničenost na incest (DSM V, 2014). Cilj ovog rada je kroz korištenje PRISMA modela analizirati postojeću literaturu u razdoblju od 2010. do 2022. godine pretražujući Web of Science bazu podataka te sažeti aktualna saznanja o pedofiliji kao psihijatrijskom poremećaju. Spoznaje o etiologiji pedofilije govore u prilog tome da je riječ o multifaktorskom poremećaju koji je najvjerojatnije uvjetovan kombinacijom okolinskih i bioloških čimbenika, s posebnim naglaskom na utjecaj malformacija na frontalnom i temporalnom korteksu u biološkom smislu, te utjecajem iskustva zlostavljanja u djetinjstvu u psihosocijalnom. U fenomenološkom smislu, predstavljene su brojne karakteristike koje se povezuju uz pedofilične osobe od kojih su najprominentnije psihijatrijski komorbiditet, hiperseksualnost, poteškoće sa zadržavanjem pažnje te poteškoće u kontroli seksualnih poriva. Prevencijske i tretmanske opcije su se pokazale nedostatnima zbog neuvažavanja potreba pedofiličnih osoba. Ova nedostatnost povezana je sa visokom stigmatizacijom ovog poremećaja koja onemogućava usmjeravanje resursa i napora prema osmišljavanju boljih intervencija. Potrebna je edukacija društva o pedofiliji i pedofiličnom poremećaju te njezino razgraničavanje od seksualnog zlostavljanja djece. Također, u radu se pedofilija promatra kroz prizmu seksualne orijentacije te se raspravlja o tome hoće li ju društvo ikada moći prihvatiti kao jednaku svim drugim seksualnim orijentacijama ili će pitanje moralnosti zauvijek zatvarati ta vrata?
Abstract (english) According to the DSM V classification, pedophilia, or more precisely, pedophilic disorder, is defined as repeated, strong sexually arousing fantasies, sexual urges or behaviors that include sexual activity with a pre-pubescent child (generally age 13 and younger) and are present for at least 6 months. Additionaly, it causes significant suffering and interpersonal difficulties for the person suffering from the mentioned disorder and have certain specificities such as exclusivity, gender of the child and limitation to incest (DSM V, 2014). The aim of this paper is to analyze the existing literature from 2010 to 2022 using the PRISMA model by searching the Web of Science database and to summarize current knowledge about pedophilia as a psychiatric disorder. Knowledge about the etiology of pedophilia supports the fact that it is a multifactorial disorder that is most likely conditioned by a combination of environmental and biological factors, with special emphasis on the influence of malformations on the frontal and temporal cortex in the biological sense, and the influence of the experience of abuse in childhood in the psychosocial sense. In the phenomenological sense, numerous characteristics are presented that are associated with pedophilic persons, the most prominent of which are psychiatric comorbidity, hypersexuality, difficulties with maintaining attention and difficulties in controlling sexual urges. Available prevention and treatment options have proven to be insufficient, not respecting the needs of pedophiliacs. This inadequacy is related to the high stigmatization of this disorder, which makes it impossible to direct resources and efforts towards designing better interventions. It is necessary to educate society about pedophilia and pedophilic disorder and distinguish it from sexual abuse of children. Also, the paper looks at pedophilia through the prism of sexual orientation and discusses whether society will ever be able to accept it as equal to all other sexual orientations, or will the question of morality close that door forever?
