Abstract
Karakteristike glasa svake pojedine osobe mijenjaju se tijekom života reflektirajući promjene uzrokovane starenjem i/ili patološkim promjenama organskih sustava koji sudjeluju u proizvodnji glasa i govora: respiratornog, fonacijskog i artikulacijskog. Svaka promjena u glasu ne znači odmah i prisutnost poremećaja glasa. Starenje glasa obično počinje nakon navršenih 65 godina života i sastavni je dio starenja organizma. Poremećaji glasa kod osoba treće životne dobi također mogu biti posljedica procesa starenja organizma ili bolesti i patoloških procesa. Ukoliko su promjene u kvaliteti glasa uzrokovane procesom starenja nazivaju se prezbifonijom, a promjene na larinksu koje su uzrokovane starenjem te dovode do prezbifonije nazivaju se prezbilarinksom. Dijagnoza prezbifonije i prezbilarinksa postavlja se tek nakon multidisciplinarne, pažljive i sveobuhvatne procjene glasa tijekom koje nije bilo moguće utvrditi neku drugu patološku organsku podlogu ili bolest laringealnog sustava koja bi bila uzrok poremećaja glasa kod osobe. U simptome prezbifonije ubrajaju se promjena visine glasa, smanjenje glasnoće, pojava šumnosti i promuklosti, slabost glasa, tremor, nemogućnost održavanja fonacije i promjena kvalitete glasa. Poremećaj glasa kod osoba treće životne dobi ima negativan utjecaj na psihosocijalnu aktivnost, emocionalno zdravlje i sveukupnu kvalitetu života osobe. Pošto se broj osoba treće životne dobi u Hrvatskoj i u svijetu sve se više povećava, prepoznavanje i terapija poremećaja glasa u ovoj populaciji postaje sve važnije. Terapiji se pristupa multidisciplinarno. Vokalna terapija prva je metoda izbora, a dosadašnja istraživanja govore u prilog njezinoj efikasnosti. Ukoliko se ona pokaže nedovoljno učinkovitom, pribjegava se kirurškim pristupima. Cilj ovog preglednog rada je prikazom osnovnih podataka o tome što je starenje i koje su normalne biološke promjene uzrokovane starenjem pokazati kako one utječu na kvalitetu glasa kod osoba treće životne dobi. Nadalje, prikazom recentnih istraživanja nastoji se prikazati koja je prevalencija i incidencija ovog poremećaja u populaciji te izdvojiti što se točno smatra poremećajem glasa kod osoba treće životne dobi i zašto kod osoba treće životne dobi dolazi do pojave poremećaja glasa. Također se navodi koje je sve postupke potrebno provesti prilikom procjene glasa te se objašnjavaju terapijske metode koje se primjenjuju u tretmanu poremećaja glasa kod osoba treće životne dobi i njihova učinkovitost.
Abstract (english)
The human voice undergoes changes over the life span associated with normal development and aging and/or pathological changes in subsystems responsible for voice and speech production (respiration, phonation and articulation). Each change does not immediately indicate presence of the voice disorder. Vocal aging generally begins after the age of approximately 65 years as part of a normal physiological aging. Voice disorders in elderly can be result of the aging process itself or may be caused by some kind of illness or pathological process. Presbyphonia is a term used to describe changes in vocal quality associated with aging process, and presbylarynx is a condition in which age-related changes of the larynx results in presbyphonia. These diagnoses can be made only after careful multidisciplinary examination and exclusion of other pathological changes and possible causes of voice disorder. Some of the symptoms of presbyphonia are changes in vocal pitch, reduced loudness, increased breathiness and hoarsness, weakness of the voice, vocal tremor, inability to sustain phonation and reduced vocal quality. The impact of voice disorders in elderly has been shown to cut across a number of quality-of-life domains such as social interaction, general and emotional health, and psychological well-being. The number of people above the age of 65 is increasing rapidly worldwide and the issue of recognizing and treating voice disorders in this population is becoming increasingly important. Therapy of presbyphonia must be multidisciplinary. Vocal therapy is considered as first method of choice and its efficiency is proven by current researches. Surgical approaches are used if vocal therapy is not showing expected results. Goal of this review is to present basic information about aging and normal biological changes in the human body caused by the aging process and to show how this changes affect voice quality in the elderly. Furthermore, prevalence and incidence, definition and causes of voice disorders in elderly are shown by presenting results of the recent researches. This paper also describes diagnostic procedures and current treatments for voice disorders in elderly and addresses its efficiency.
