Abstract
Nakon moždanog udara, poremećaji jezika jedni su od najčešćih nemotoričkih posljedica koji se javljaju u čak 35% bolesnika. Klinička slika jezičnih poremećaja vrlo je varijabilna, te oni u mnogome onesposobljavaju bolesnika s moždanim udarom, često i više nego sam motorički deficit. U starijoj literaturi zabilježen je podatak kako moždani udari u desnoj hemisferi za posljedicu nemaju jezični poremećaj jer je jezik lokaliziran u lijevoj hemisferi. No, današnje spoznaje uvelike se razlikuju. Razvojem, logopedije i medicine te posebno tehnologije, sve više se govori o tome kako jezik uključuje kompletnu neuralnu mrežu. Istraživanja pomoću funkcionalne magnetske rezonancije (fMRI) otvorila su vrata novom shvaćanju funkcioniranja mozga pri obradi jezika i govora. Naime, ovisno o vrsti jezičnih zadataka ponekad je aktivna lijeva, a ponekad desna hemisfera, no u većini jezičnih djelatnosti sudjeluju obje hemisfere. Također, klinička praksa svjedok je bolesnicima čije jezične dijagnoze ne odgovaraju lokalizacijama njihovog mozgovnog oštećenja. Iz tog razloga, staro načelo jedna lokalizacija-jedna dijagnoza sve više se smatra nedostatnim. Kakve reperkusije na jezik imaju desnostrani moždani udari i razlikuju li se oni kvantitativno i kvalitativno od lijevostranih moždanih udara u jezičnoj proizvodnji pitanja su koja će se pokušati odgovoriti provedbom ovog diplomskog istraživanja. Cilj je opisati jezičnu proizvodnju u bolesnika s moždanim udarom u desnoj hemisferi u akutnoj fazi bolesti (1-5 dana nakon nastanka moždanog udara) te je usporediti s bolesnicima s moždanim udarom u lijevoj hemisferi i kontrolnim ispitanicima. Pretpostavka je da se jezični profili bolesnika s desnostranim i lijevostranim moždanim udarom neće razlikovati. U ovom istraživanju sudjelovalo je 10 bolesnika s moždanim udarom u desnoj hemisferi, a bili su ispitani pomoću Sveobuhvatnog testa za procjenu afazije (Swinburn, Potter, Howard, Kuvač Kraljević, Lice i Matić 2019). Rezultati su potvrdili da se bolesnici s lijevostranim i desnostranim moždanim udarima ne razlikuju statistički značajno u gotovo svim varijablama jezične proizvodnje, čime se potvrđuje pretpostavka da će moždani udari u desnoj hemisferi također za posljedicu imati jezične teškoće.
Abstract (english)
After a storke, language disturbances are one of the most common nonmotoric deficits that occur in about 35% of the patients. Clinical features of language disturbances are very variable and they significantly limit the life of a stroke patient even more than the motoric deficit. In older literature there is a register of data that says that strokes in the left hemisphere do not result in language disturbances because language is localised in the left hemisphere. However, today's knowledge is greatly different. With the development of speech language pathology, medicine and especially technology, it has been emphasized that language is spread on the whole neural network. Research with functional magnetic resonance imaging (fMRI) have opened the door towards new understanding of brain functioning whilst processing speech ang language. Depending on the type of language task, sometimes the left hemisphere is active, sometimes the right, but in most tasks, both hemispheres are active. What are the repercusions of right hemisphere strokes on languag and do they qualitatively and quantitatively differ from left hemisphere strokes in language production are questions with the intention of finding an answer conducting this master research. The aim is to describe language production in patients with right hemisphere stroke in the acute phase of disease (1-5 days after stroke) and compare it with patients with left hemisphere stroke and the control group. The assumption is that language profiles of patients with right hemisphere strokes and patients with left hemisphere strokes won't differ. In this research 10 patients with right hemisphere strokes were assesed using Comprehensive Aphasia Test (Swinburn, Potter, Howard, Kuvač Kraljević, Lice i Matić 2019). The results showed that there was no significant difference between patients with right and left hemispheric strokes on almost all parametres of language production, which confirms the assumption that right hemispheric strokes patients will also have language disturbances.
