Abstract Karijes je jedna od najčešćih kroničnih infekcijskih bolesti koja pogađa ljude diljem svijeta.
Zahvaća tvrda zubna tkiva i predstavlja nepovoljni rezultat dinamične izmjene procesa
demineralizacije i remineralizacije, odnosno nastaje kad prevlada demineralizacija.
Demineralizacija je otapanje tvrdog zubnog tkiva, a remineralizacija je obnova komponenti
izgubljenih tijekom demineralizacije. Kako bi se spriječilo stvaranje ili zaustavilo napredovanje
početne karijesne lezije važni su preventivni i interceptivni postupci. Odnose se na remineralizaciju
zubnog tkiva, odnosno fluoridaciju u užem smislu. Fluoridi su se pokazali vrlo učinkovitim
sredstvom u prevenciji karijesa te se mogu primijeniti sistemski ili topikalno. Sistemska primjena
danas se sve više napušta zbog rizika od dentalne fluoroze, a uključuje konzumaciju fluoridirane
vode, soli ili mlijeka, te unos tableta fluora. Topikalna fluoridacija može se provoditi u obliku
zubnih pasti i tekućine za ispiranje usta, u obliku gelova, lakova, otopina, dražeja za otapanje u
ustima, žvakaćih guma, naprava za otpuštanje fluora te restaurativnih materijala koji imaju
mogućnost otpuštanja fluora. S obzirom na kemijski sastav spojeve fluora u preparatima za
topikalnu fluoridaciju dijelimo na anorganske i organske. Anorganski spojevi su natrijev fluorid,
natrijev monofluorofosfat, kositreni fluorid, zakiseljeni preparati fluora i titanov tetrafluorid, a
organski su aminfluorid i SDF. Osim preparata fluora za remineralizaciju se koriste i sredstva koja
su temeljena na kalcijevu fosfatu i hidroksiapatitu. Spojevi kalcijeva fosfata su ACP, CPP-ACP i
bioaktivno staklo. SDF se pokazao kao najbolje sredstvo u prevenciji karijesa, nakon njega najbolje
rezultate daju preparati s NaF-om, pa onda preparati koji sadrže CPP-ACP.
Abstract (english) Caries is one of the most common chronic infectious diseases affecting people around the world.
It affects the hard tooth tissue and it presents an unfavorable result of dynamic changes in the
process of demineralization and remineralization, ie it occurs when demineralization prevailed.
Demineralization is the dissolution of hard tooth tissue, and remineralization is the restoration of
components lost during demineralization. Preventive and interceptive procedures are important to
prevent the formation or stop the progression of the initial carious lesion. They refer to the
remineralization of dental tissue, ie fluoridation in the narrow sense. Fluorides have proven to be a
very effective tool in caries prevention and can be applied systemically or topically. Systemic use
is increasingly being abandoned today due to the risk of dental fluorosis, and includes the
consumption of fluoridated water, salt or milk, and the intake of fluoride tablets. Topical
fluoridation can be carried out in the form of toothpastes and mouthwashes, in the form of gels,
varnishes, solutions, mouthwashes, chewing gum, fluoride release devices and restorative materials
capable of releasing fluoride. According to the chemical composition of fluorine compounds in
preparations for topical fluoridation, they are divided into inorganic and organic. Inorganic
compounds are sodium fluoride, sodium monofluorophosphate, tin fluoride, acidified fluorine
preparations and titanium tetrafluoride, and organic are amine fluoride and SDF. In addition to
fluorine preparations for remineralization, agents based on calcium phosphate and hydroxyapatite
are also used. Calcium phosphate compounds are ACP, CPP-ACP and bioactive glass. SDF has
been shown to be the best agent in caries prevention, followed by NaF agents and then CPP-ACP
containing agents.
