Abstract U okviru disertacije je provedena analiza pristupa ispitanica mjernom stolu za potrebe medicinske pretrage ultrazvukom odnosno probira radi raka dojke, ponajprije s namjerom utvrđivanja utjecaja razlika osobina i sposobnosti ispitanica te uvjeta izvođenja kretnji pristupa, kako na trajanje pretrage, tako i na utvrđivanje značajki najpovoljnijeg konstruktivnog rješenja takvog sustava. Uzimajući u obzir polaznu zamisao i uočene probleme, provedena analiza obuhvaća utvrđivanje značajki mišićnog opterećenja, utjecaj antropomjera, uvjete korištenja mjernog stola, trajanje razmatranih kretnji i na kraju procjenu zauzetog željenog stava za pretragu. Analiza se provodi na tri modela, odnosno na tri rješenja mjernog stola i pripadnih kretnji pristupa. Obzirom da je riječ o različitim utjecajnim veličinama, za objektivniju prosudbu uvedena je izvorna metoda procjene ergonomičnosti, koja za svaki od razmatranih modela omogućava tvorbu jedinstvene ocjene na temelju postavljenog kriterija ergonomičnosti. Uvedena metoda omogućava obuhvaćanje ocjena svih značajki uzetih u razmatranje, neovisno o njima pripadnoj mjernoj jedinici, odnosno bezdimenzijski. Time se u primjeni ergonomije po prvi puta otvara mogućnost da se ostvarene kvalitete iskažu brojčano, što nedvojbeno tvori objektivnu sliku o razmatranim rješenjima, pored mogućnosti izravne usporedbe načelno različitih primjera. Iz ovakve analize proizlazi idejno rješenje pristupa koji podrazumijeva naslanjanje na stol modificiran za pretragu, što znači da nije potrebno provoditi složeno konstruiranje novog stola već izvršiti potrebne preinake postojećih. Osim toga, utvrđeno je da je moguća primjena silikonskog međusloja, iako ultrazvuk još nije stasao u samostalnu dijagnostičku tehnologiju za probir. Kao utjecajna veličina pri konstruiranju mjernog stola i utvrđivanja mogućnosti automatizacije pretrage, antropometrija gornjeg trupa ispitanica (grudi i s njima povezane izmjere) se smatra nezavisnom značajkom spram drugih dimenzija tijela, te nije utvrđen način da se predvidi raspodjela ovih dimenzija u ovisnosti o visini ili drugim vezanim dimenzijama. Na temelju izrečenog, iako je utvrđen najpovoljniji odnos ispitanica i mjernog sustava po pitanju pristupa, te se posredni pregled ultrazvukom može ostvariti, glavnu zapreku u ostvarenju zamisli kratkotrajnog automatiziranog probira ultrazvukom predstavlja upravo primjena ultrazvuka, koji ne omogućava pouzdanu samostalnu primjenu za probir radi raka dojke.
Abstract (english) In order to provide quick and automated medical ultrasound screening procedure for early detection of breast cancer, the main goal of dissertation is to extract the best biomechanical and ergonomic relations between women and diagnostic system. Since women should approach to the diagnostic system (special worktable), in order to provide access to breast tissue ultrasound scan, they should perform related movements with concluding posture, demonstrated by three different models. Identification of various differences of women abilities, anthropometrical data and others that are related with required approach movements, as well as desired shortest period for their completion, it affects that analysis includes following factors: caused muscular effort, anthropometrical data influence, worktable exploitation terms, related movements duration and ergonomic posture estimation. Objectivity of different factors estimation implies introduction of original ergonomic estimation method with the idea of forming unique non-dimensional numerical mark for each model, based on ideal ergonomic criteria. This method offers not only comparison capability for models, but also noticing their advantages through estimated factors. Analysis implies bending posture over modified worktable, which offers the best ergonomic estimation mark and excludes new worktable construction. Besides that, the use of silicon differentiate layer was confirmed for indirect ultrasound scan, although ultrasound cancer screening is not yet verified. Further on, women upper trunk anthropometric data are independent, without known correlation with other body measures or their prediction approach. Based on analysis results, which reveal the most recommended biomechanical and ergonomic relations between women and diagnostic system as well as confirmation for use of silicon differentiate layer, ultrasound diagnostic technology is still the main obstructive for realization of quick and automated medical ultrasound screening procedure for early, reliable and self-sufficient detection of breast cancer.
