Abstract U populaciji pacijenata preživjelih od moždanog udara, kontrola ravnoteže predstavlja izazov s obzirom na posljedičnu manifestaciju hemipareze. Više od 50% preživjelih osoba doživi pad unutar jedne godine nakon moždanog udara (npr. Ashburn i sur., 2008). Procjena ravnoteže kod pacijenta s moždanim udarom potrebna je kako bi se odredili objektivni ciljevi i metode liječenja te kako bi se omogućilo praćenje napretka i predviđanje prognoze. Osim toga, korištenje objektivnih alata za mjerenje ravnoteže važno je za praksu utemeljenu na dokazima kako bi se poboljšala sposobnost ravnoteže. Glavni cilj istraživanja je utvrditi utjecaj MU na balans kod osoba koje su ga preboljele. Specifičan cilj istraživanja je utvrditi utjecaj MU na balans i procijeniti rizik od pada kod osoba koje su ga preboljele na temelju procjene statičkih i dinamičkih funkcionalnih zadataka. Istraživanje je provedeno na temelju dokumentacije pacijenata prikupljene u vremenskom periodu od 2016. godine do 2021 godine. U istraživanje je bilo uključeno 30 ispitanika liječenih stacionarno na odjelu Zavoda za fizikalnu medicinu i rehabilitaciju Kliničkog bolničkog centra u Rijeci. Procjena balansa i rizika od pada provedena je Berg-Balance skalom (engl. Berg-Balance Scale, BBS). Test se boduje ljestvicom od 0 do 4, pri kojoj 0 označava nemogućnost izvršenja zadatka a 4 potpunu neovisnost pri izvršavanju zadatka. Ukupan broj bodova ljestvice iznosi 56. Granične vrijednosti za starije osobe prema Bergu i sur. (1992) podrazumijevaju 56 i 45 bodova na ljestvici. Ocjena 56 pokazatelj je optimalne funkcionalne ravnoteže dok Ocjena < 45 ukazuje na to da pojedinci mogu biti u većem riziku od pada (75). U istraživanju je sudjelovalo 30 ispitanika, od kojih 76,67% (n=23) ispitanika muškog spola i 23,33% (n=7) ispitanika ženskog spola. Prosječna dob ispitanika iznosila je 67,10 ± 8,14 godina, a medijan 66 godina. Ispitanici su započeli rehabilitaciju, odnosno napravili početno mjerenje prosječno 38,53 ± 29,41 dana nakon moždanog udara, najbrže nakon 20 dana, a najkasnije 161 dan. 53,33% (n=16) ispitanika imalo je zahvaćenu desnu stranu, a 46,67% (n=14) lijevu stranu. Ispitanici su u prosjeku postigli 19,13 ± 10,73 bodova na BBS-u. Najmanji postignuti rezultat iznosio je 2 boda, a najveći 40. Samim time svi ispitanici postigli su manju vrijednost od postavljene granične vrijednosti (<45 bodova) koja ukazuje na povećani rizik od pada. Rezultati istraživanja potvrdili su da osobe koje su preboljele MU imaju statistički značajno narušen balans i pokazuju povećani rizik od pada.
Abstract (english) In stroke patients, balance control is a challenge given the consequent manifestation of hemiparesis. More than 50% of survivors experience a balance deterioration within one year after stroke (e.g., Ashburn et al., 2008). Balance assessment in a stroke patient is needed in order to determine objective goals and treatment methods, and to allow progress to be monitored and prognosis to be predicted. In addition, use of objective balance measurement tools is important for evidence-based practice in order to improve balance ability. The main goal of the study is to determine the impact of stroke on balance in stroke survivors. The specific goal of the study is to determine the impact of stroke on balance and to assess the risk of falling in stroke survivors, based on the assessment of static and dynamic functional tasks. The study was conducted on the basis of medical records collected in the period from 2016 to 2021. The study included 30 subjects undergoing inpatient treatment at the Department of Physical Medicine and Rehabilitation of the Clinical Hospital Centre in Rijeka. The assessment of balance and risk of falling was performed using the Berg-Balance Scale (BBS). The test is scored on a scale from 0 to 4, where 0 indicates the impossibility of performing the task and 4 total independence in performing the task. The total number of points on the scale is 56. Limit values for the elderly, according to Berg et al. (1992), imply 56 and 45 points on the scale. A score of 56 is an indicator of optimal functional balance while a score of <45 indicates that individuals may be at higher risk of falling (75). Thirty subjects participated in the study, of which 76.67% (n = 23) were male and 23.33% (n = 7) female. The average age of the subjects was 67.10 ± 8.14 years, and the median was 66 years. Rehabilitation started, i.e., the subjects made an initial measurement on average 38.53 ± 29.41 days after stroke, the fastest after 20 days, and no later than 161 days. In 53.33% (n = 16) of the subjects the right side was affected, and in 46.67% (n = 14) the left side. In average, the subjects scored 19.13 ± 10.73 points on the BBS. The lowest score was 2 points and the highest was 40. Thus, all subjects achieved a lower value than the set limit value (<45 points), which indicates an increased risk of falling. The results of the study confirmed that stroke survivors have a statistically significantly disturbed balance and show an increased risk of falling.
