Abstract Hernija lumbalnog diska jedan je od čestih razloga subakutne i kronične boli u lumbosakralnoj kralježnici. Ovisno o volumenu herniranog materijala može doći do kompresije spinalnog živca ili kralježničke moždine, što uzrokuje kompleksnije simptome poput radikularne boli, parestezija i mišićne slabosti. Lumbalna mehanička trakcija fizikalna je procedura koja se od davnina koristi s ciljem liječenja problematike intervertebralnih diskova i boli u donjem dijelu leđa, pretežito u kombinaciji s ostalim fizikalnim modalitetima. Podrazumijeva istezanje segmenata kralježnice i okolnih pripadajućih struktura uslijed čega dolazi do separacije kralježaka i dekompresije živaca ili leđne moždine. Cilj istraživanja bio je ispitati kratkoročnu učinkovitost mehaničke lumbalne trakcije na smanjenje subakutne boli i stupnja funkcionalne onesposobljenosti kod osoba s dijagnosticiranom hernijom lumbalnog intervertebralnog diska. U istraživanju je sudjelovalo 20 ispitanika u dobi od 18 do 65 godina čija je kvaliteta života smanjena utjecajem simptoma diskus hernije. Svi ispitanici sudjelovali su u identičnom rehabilitacijskom programu koji se sastojao od 20 minuta Tecar terapije i statičke trakcije na Massotrax stolu, 15 minuta intermitentne trakcije u supiniranom položaju te 15 minuta magnetoterapije, a sve kroz 3 tretmana. Ispitanici su bili podvrgnuti testiranju netom prije i 72 sata nakon rehabilitacijskog ciklusa. Glavne mjere ishoda koje su se analizirale bile su intenzitet boli mjeren VAS skalom te stupanj funkcionalne onesposobljenosti bodovan Oswestry upitnikom. Nakon što su prikupljeni podaci, statističkom analizom podataka utvrđeno je smanjenje intenziteta boli u području lumbosakralne kralježnice za 50 posto u odnosu na stanje prije terapije, dok je intenzitet boli u predjelu donjeg ekstremiteta smanjen za čak 64 posto. Jednako tako, povoljni rezultati dobiveni su i po pitanju stupnja funkcionalne onesposobljenosti, gdje je ista nakon tri tretmana mehaničkom lumbalnom trakcijom, s prosječnih 50% snižena na 26%. Drugim riječima, ispitanici su prije terapije bili teško onesposobljeni za obavljanje svakodnevnih životnih aktivnosti, dok su rezultati nakon terapije ukazali na umjerenu onesposobljenost. Dobiveni rezultati upućuju na to da je terapija mehaničkom lumbalnom trakcijom ukazala na značajno smanjenje intenziteta boli i stupnja invalidnosti, čime se može zaključiti kako je predmetna metoda učinkovita u unaprjeđenju kvalitete života pojedinca i samim time poželjna metoda u liječenju problematike intervertebralnih diskova. S obzirom na to da je dokazana kratkoročna učinkovitost, potrebna su daljnja istraživanja koja će dodatno istražiti postojanje dugoročnog učinka te podrobnije analizirati parametre trakcije koji su potrebni za što uspješniji tijek liječenja osoba s hernijacijom intervertebralnog diska.
Abstract (english) Lumbar disc herniation is common cause of subacute and chronic pain in the lumbosacral spine. Depending on the volume of the herniated material, compression of the spinal nerve or spinal cord can occur which causes complex symptoms such as radicular pain, paraesthesia and muscle weakness. Lumbar mechanical traction is a physical procedure that has been used for a long time to treat intervertebral disc problems and lower back pain, mostly in combination with other physical modalities. It involves stretching the segments of the spine and the surrounding associated structures, resulting in the separation of the vertebrae and decompression of the nerves or spinal cord. The aim of the paper is to examine the short-term efficacy of mechanical lumbar traction in reducing subacute pain and the level of functional disability in persons diagnosed with lumbar intervertebral disc herniation. The study involved 20 subjects aged 18 to 65 years whose quality of life was reduced by the influence of disc herniation symptoms. All subjects participated in an identical rehabilitation program consisting of 20 minutes of Tecar therapy and static traction on a Massotrax table, 15 minutes of intermittent traction in the supinated position, and 15 minutes of magnetotherapy through 3 treatments. Subjects underwent testing before and 72 hours after the rehabilitation cycle. The main outcome measures that were analyzed were pain intensity measured by the VAS scale and the level of functional disability scored by the Oswestry questionnaire. After the data has been collected, statistical analysis showed a reduction in pain intensity in the lumbosacral spine by 50 percent compared to the condition before therapy, while the intensity of pain in the lower extremity was reduced by 64 percent. Equally, favorable results were obtained in terms of the level of functional disability where, after three treatments with mechanical lumbar traction, mentioned disability was reduced from an average of 50% to 26%. In other words, before therapy, subjects were severely disabled to perform daily life activities while results after therapy indicated moderate disability. These results showed that mechanical lumbar traction therapy indicated a significant reduction in pain intensity and level of disability, which leads to the conclusion that the method is effective in improving the quality of life of an individual and thus a desirable method in treating intervertebral disc problems. Given that only short-term efficacy has been demonstrated, further research is needed to investigate the existence of a long-term effect and to analyze in detail the traction parameters required for the most successful course of treatment of persons with intervertebral disc herniation.
