Abstract Uvod: Ugradnja totalne artroplastike zgloba kuka najučestalija je operacija današnjice. Uspješan operativni zahvat uz kvalitetnu fizioterapijsku rehabilitaciju rezultira boljom kvalitetom života i bržim povratkom aktivnostima svakodnevnog života. Oporavak karakteriziraju smanjena mišićna jakost, bol i funkcionalna ograničenja pri čemu je najučestalija slabost abduktorne muskulature kuka. Antigravitacijska traka je pokretna traka koju čini sustav diferenciranog tlaka zraka unutar balona koji omogućuje smanjenje sile gravitacije i težine tijela tokom aktivnosti. Dostupna istraživanja ukazuju na pozitivne učinke antigravitacijske trake na mišićnu jakost i izdržljivost, bol, opseg pokreta, potrošnju kisika i funkcionalnu mobilnost. Sukladno tome, cilj ovog istraživanja bio je ispitati učinak antigravitacijske trake na jakost abduktorne muskulature, opseg pokreta zgloba kuka, bol i funkcionalnu mobilnost nakon totalne artroplastike zgloba kuka.
Ispitanici i metode: U istraživanju je sudjelovalo 34 ispitanika nasumično podijeljenih u dvije skupine: eksperimentalnu i kontrolnu. Eksperimentalna skupina imala je dodatak vježbi na antigravitacijskoj traci u trajanju od 20 minuta, dok je ostala fizioterapijska intervencija bila jednaka za obje skupine. Mjerene varijable su jakost abduktorne muskulature, opseg pokreta, bol i razina funkcionalne mobilnosti. Za izračun varijabli koristili smo dinamometar, ručni goniometar, brojčanu skalu boli, te TUG i 40mFPWT za procjenu funkcionalne mobilnosti.
Rezultati: Dobiveni rezultati ukazuju na postojanje statistički značajne razlike u završnim mjerenjima pokreta abdukcije između skupina u korist eksperimentalne skupine (p = 0,044). Nadalje, nije dokazana statistički značajna razlika između skupina u ostalim izmjerenim varijablama.
Zaključak: Istraživanjem je utvrđen pozitivan učinak antigravitacijske trake na opseg pokreta zgloba kuka. S obzirom da je antigravitacijska traka novost u rehabilitacijskom protokolu nakon ugradnje umjetnog zgloba kuka potrebna su daljna istraživanja na ovu temu, a sve s ciljem sigurnijeg, kvalitetnijeg i bržeg povratka aktivnostima svakodnevnog života s posljedičnim manjim troškom na zdravstveni sustav.
Abstract (english) Introduction: Implementation of total hip joint arthroplasty is the most common operation today. A successful operation with quality physiotherapy rehabilitation results in a better quality of life and faster return to the everyday life activities. Recovery is initially characterized by reduced muscle strength, pain and functional limitations but the most common is weakness of the hip abductor muscles. The antigravity treadmill is a moving band made up of a system of differential air pressure inside the balloon that enables the reduction of the force of gravity and the weight of the body during activity. Available research indicates positive effects of antigravity treadmill on muscle strength, pain, endurance, range of motion, oxygen consumption and functional mobility. Accordingly, the aim of this study was to examine the effect of the antigravity band on the strength of the abductor musculature, the range of movement of the hip joint, pain and functional mobility after total arthroplasty of the hip joint.
Participans and methods: This research included 34 subjects, randomly divided into two groups: experimental and control. The experimental group had the addition of an antigravity treadmill exercise for 20 minutes, while the rest of the physiotherapy intervention was the same for both groups. The measured variables are the strength of the abductor muscles, the range of motion, pain and the level of functional mobility. To calculate the variables, we used a dynamometer, a manual goniometer, a numerical pain scale, and TUG and 40mFPWT tests for assessing functional mobility.
Results: The obtained results indicate the existence of a statistically significant difference in the final measurements of abduction movements between groups in favor of the experimental group (p = 0.044). Furthermore, no statistically significant difference was demonstrated between groups in other measured variables.
Conclusion: The study found a positive effect of the antigravity treadmill on the range of motion of the hip joint. With consideration that the antigravity treadmill is a novelty in the rehabilitation protocol after implementation of the arthroplasty of the hip joint, further research on this theme is needed, all with the aim of a safer, better and faster return to the activities of everyday life with the consequent lower cost to the health system.
