Abstract Najčešći patološki sadržaj u prsištu u djece su zrak (pneumotoraks) i parapneumonični izljev, a liječe se najčešće pleuralnom drenažom.
Cilj istraživanja: utvrditi broj ispitanika kojima je bio postavljen pleuralni dren, koje su bile indikacije, tehnike postavljanja i veličinu postavljenih pleuralnih drenova. Ostali ciljevi bili su analizirati dodatne specifične terapijske postupke koji su se provodili tijekom pleuralne drenaže, utvrditi nalaze mikrobiološke analize vrha pleuralnog drena i punktata, učestalost komplikacija u ispitanika s pleuralnom drenažom, te ishode liječenja.
Metode i ispitanici: Ispitanici su sva djeca od rođenja do 18. godine s patološkim sadržajem u prsištu zaprimljena na Odjel intenzivnog liječenja djece Kliničkog bolničkog centra Rijeka u razdoblju od 01.01.2014. do 31.12.2018, a imala su postavljen pleuralni dren. Podatci su prikupljeni iz arhivirane medicinske dokumentacije, integriranog bolničkog informacijskog sustava i protokola za prijem bolesnika.
Rezultati istraživanja: Pleuralni dren postavljen je u 93 djece, a drenaža je provedena kod 53 djevojčice i 40 dječaka. Indikacije za postavljanje pleuralnog drena bili su parapneumonični izljev, pneumotoraks, hemotoraks, hilotoraks i operativni zahvati prsišta. Najčešća tehnika postavljanja pleuralnog drena bila je putem troakara, potom Seldingerova tehnika, dok je najčešće postavljan dren veličine 12 Fr. U nekih bolesnika provodila se repunkcija prsišta, revizija položaja postojećeg drena i fibrinolitička terapija.
Zaključak: Najčešća indikacija za postavljanje pleuralnog drena u djece je parapneumonični izljev, a aktivna pleuralna drenaža je sigurna i učinkovita metoda liječenja patološkog sadržaja u prsištu u djece.
Abstract (english) The most common pathological contents in the chest in children are air (pneumothorax) and parapneumonic effusion. They are most commonly treated with pleural drainage.
Aim of the study: To determine the number of subjects who had pleural drainage, what were the indications, placement techniques and size of pleural drains. Other objectives were to analyze additional specific therapeutic procedures performed during pleural drainage, to determine the findings of microbiological analysis of the tip of the pleural drains, the frequency of complications in subjects with pleural drainage, and treatment outcomes.
Methods and subjects: All children from birth to 18 years with pleural drainage admitted to Pediatric Intensive Care Unit, Clinical Hospital Center Rijeka in the period from 01.01.2014. – 31.12.2018, were included in the study. Data were collected from archived medical records, integrated hospital information system and patient admission protocols.
Results of the study: Pleural drainage was set in 93 children (53 girls and 40 boys). Indications for pleural drainage placement were parapneumonic effusion, pneumothorax, hemothorax, chylothorax and chest surgery. The most common technique for placement of the pleural drain was via trocar, after that the Seldinger technique. The most common drain was the size 12 Fr. Repunction, revision of the position of the existing drainage and fibrinolytic therapy were performed in a number of patients.
Conclusion: The most common indication for placement of pleural drainage in children is parapneumonic effusion. Active pleural drainage is a safe and effective method of treating pathological content in the chest in children.
