Abstract Koljeno je najveći zglob u ljudskome tijelu i zbog velikog opterećenja sklono je učestalim sindromima prenaprezanja ili kroničnim oštećenjima. Patelarni tendinitis, kao jedan od najčešćih sindroma prenaprezanja u vrhunskom i rekreativnom sportu, predstavlja veliki izazov za liječenje. Karakterizira ga bol lokalizirana na inferiornom polu ivera nastala radi akumuliranog stresa na patelarnu tetivu. Najčešće se pojavljuje kod skakačkih sportova, češće kod muškaraca. Dovodi do prestanka sportske aktivnosti, a u nekim slučajevima i kraja sportske karijere. Dijagnostika se temelji na detaljnom fizičkom pregledu, uzimajući u obzir vanjske i unutarnje rizične faktore nastanka. Sam proces rehabilitacije najčešće je dug i može biti frustrirajući. Liječenje je u većini slučajeva konzervativno, a kineziterapija, kao jedna opcija konzervativne metode, postaje izuzetno popularna u tretiranju ovog sindroma prenaprezanja. Operativno liječenje najčešće je zadnja opcija. Izolirane ekscentrične
kontrakcije dugo vremena slove kao najefikasnija i najpopularnija metoda liječenja. Međutim, radi popratne boli koja se često pojavljuje u primjeni ekscentričnih kontrakcija, provodi se niz novih istraživanja, te se dolazi do novih metoda u liječenju patelarnog tendinitisa. Heavy Slow Resistance metoda, Silbernagelova metoda i metoda izometričke kontrakcije danas
predstavljaju kvalitetnu i manje invazivnu alternativu u kineziterapijskom liječenju. Iako još uvijek ne postoji zlatni standard za liječenje patelarnog tendinitisa, temeljem dosadašnjih istraživanja u ovom radu predstavljen je dvanaestotjedni kineziterapijski program vježbanja za liječenje patelarnog tendinitisa košarkaša. U daljnjim se istraživanjima veći fokus mora
posvetiti drugim parametrima u samom procesu rehabilitacije, kao što su intenzitet opterećenja, brzina izvođenja, frekvencija treninga i period odmora. Istraživanjem tih parametara, lakše ćemo definirati najefikasniju metodu kineziterapijskog liječenja i što bržeg povratka sportskoj aktivnosti kao ključnog cilja rehabilitacije u profesionalnom sportu.
Abstract (english) The knee is the largest joint in the human body and due to its high load, it is prone to frequent strains of overuse or chronic damage. Patellar tendinitis, one of the most common overuse syndromes in elite and recreational sports, is a major treatment challenge. It is characterized
by pain localized on the inferior pole of the patella, as a result of accumulated stress on the patellar tendon. It most often occurs in jumping sports, frequently in men, leading to the prolonged absence of sports activity, and in some cases the end of sports career. Diagnosis is
based on a thorough physical examination, taking into account external and internal risk factors. The rehabilitation process itself is usually long and can be frustrating. Treatment is conservative in most cases and kinesitherapy, as one option of the conservative method, is becoming extremely popular in treating this overuse syndrome. Surgery is usually the last option. Isolated eccentric contractions have long been known as the most effective and popular treatment method. Due to the accompanying pain that often occurs in the application of eccentric contractions, a number of new studies are being conducted and new methods are being developed in the treatment of patellar tendonitis. The Heavy Slow Resistance method, the Silbernagel method, and the Isometric Contraction method represent quality and less invasive alternatives in kinesitherapy. Although there is still no gold standard for the treatment of patellar tendinitis, based on previous research, this paper presents a 12-week kinesitherapy exercise program for the treatment of patellar tendinitis in basketball players. In future research, greater focus must be placed on other parameters in the rehabilitation process itself, such as exercise intensity, exercise speed, training frequency, and rest period to develop the most effective method of kinesitherapy treatment. This research will make it possible for the athlete to return to sports activity as quickly as possible, a key goal of rehabilitation research in elite sports.
