Abstract Cilj ovog sustavnog preglednog rada i meta-analize bio je utvrditi veličinu akutnih učinaka ekscentričnih mišićnih akcija na osjećaj za poziciju zgloba koljena kod zdravih odraslih osoba. S tim ciljem, sustavno su pretražene tri relevantne elektroničke baze podataka: PubMed, Scopus i Web of Science. U sustavni pregled i meta-analizu uključene su studije koje su proučavale akutne učinke ekscentričnih mišićnih akcija na osjećaj za poziciju zgloba koljena mjerenog i izražavanog putem apsolutne pogreške u poziciji zgloba. Inicijalnom pretragom je dobiveno 450 studija, dok je u kvalitativnu analizu uključeno 8 studija, te je isti broj studija podvrgnut meta-analizi prema modelu slučajnih učinaka. Napravljen je niz od 6 zasebnih meta-analiza podijeljenih u odnosu na: 2 vremenske točke testiranja osjećaja za poziciju zgloba (testiranje 1 sat nakon intervencije i 1 dan nakon intervencije) i 3 podgrupe definirane prema veličini kuta fleksije u koljenu prilikom testiranja osjećaja za poziciju zgloba (mali, srednji i veliki kut). Sumarni efekti i rezultati meta-analiza prikazani su putem standardiziranih razlika aritmetičkih sredina i 95%-tnih intervala pouzdanosti SRAS (95% IP), uz razinu statističke značajnosti p < 0,001. Rezultati meta-analiza utvrdili su statistički značajno narušen osjećaj za poziciju zgloba (p < 0,001) , bez obzira na veličinu kuta i vremensku točku testiranja osjećaja za poziciju zgloba nakon ekscentričnih mišićnih akcija: 1h; mali kut: -1,55 (-2,15 do -0,95), srednji kut: -1,26 (-1,71 do -0,80), veliki kut: -1,17 (-1,63 do -0,71) / 1 dan; mali kut: -1,26 (-1,76 do -0,76) , srednji kut: -1,29 (-2,08 do -0,49) , veliki kut: -1,31 (-1,97 do -0,66). Uz meta-analize napravljene su i univarijatne meta-regresijske analize za sljedeće kovarijable: spol, ukupan broj kontrakcija, pad sile 1 sat nakon intervencije i pad sile 1 dan nakon intervencije. Meta-regresijskim analizama utvrđena je statistički značajna (p < 0,05) povezanost između pada sile 1 dan nakon intervencije i apsolutne pogreške u osjećaju za poziciju zgloba testiranog pri malom i velikom kutu, 1 dan nakon intervencije. S obzirom na robusne i konzistentne rezultate 6 meta-analiza, zaključujemo da ekscentrične mišićne akcije akutno narušavaju osjećaj za poziciju zgloba koljena.
Abstract (english) The aim of this systematic review and meta-analysis was to determine the magnitude of acute effects of eccentric muscle actions on knee joint position sense in healthy adult population. Accordingly, three relevant electronic databases were systematically searched: PubMed, Scopus and Web of Science. Studies included in this systematic review and meta-analysis were those which investigated acute effects of eccentric muscle actions on knee joint position sense measured and expressed via joint position sense absolute error. Initial search yielded 450 studies, while 8 studies were included in qualitative analysis and the same number of studies was subjected to random-effects meta-analysis. A series of 6 separated meta-analyses were done according to: 2 time points in which joint position sense was tested ( testing done at 1 hour after intervention and 1 day after intervention) and 3 subgroups which were defined according to the size of knee flexion angle in joint position sense testing protocol (small, moderate and large angle). Summary effects and results of meta-analyses were presented as standardized mean differences with 95% confidence intervals, while statistical significance was set at p < 0,001. Results of meta-analyses showed statistically significant decrease in joint position sense (p < 0,001), no matter which angle size and time point was taken into account: 1 hour; small angle: -1,55 (-2,15 to -0,95), moderate angle: -1,26 (-1,71 to -0,80), large angle: -1,17 (-1,63 to -0,71); and 1 day; small angle: -1,26 (-1,76 to -0,76), moderate angle: -1,29 (-2,08 to -0,49), large angle: -1,31 (-1,97 to -0,66). Along with meta-analyses, univariate meta-regressions were performed on next covariates: sex, total number of contractions, force drop at 1 h post intevention and force drop at 1 day post intervention. Meta-regressions revealed statistically significant (p < 0,05) relation between force drop at 1 day post intervention on joint position sense absolute error tested at small and large angles, 1 day post intervention. Considering the robust and consistent results of 6 meta-analyses, it can be concluded that eccentric muscle actions acutely decrease knee joint position sense.
