Abstract Koljeno je najkompleksniji kutno-obrtni zglob koji trpi najveća opterećenja i upravo zbog toga je sklon različitim ozljedama. Rupture prednje ukrižene sveze (engl. Anterior cruciate ligament – ACL) čine 50 % svih ozljeda koljena kod sportaša. Rehabilitacijski proces rupture ACL-a nema standardizirane protokole već okvirne smjernice za vođenje kroz rehabilitaciju koja traje šest do osamnaest mjeseci. Sa druge strane veliki zdravstveni problem u sportu čine sve učestaliji recidivi ruptura ACL-a koji se u najvećem postotku javljaju u nogometu, iako je incidencija u porastu i kod drugih sportova. Prepoznavanje rizičnih čimbenika poput neuroloških i biomehaničkih promjena, psiholoških faktora te umora ključno je u cilju smanjenja incidencije. Razdoblje između šestog i devetog mjeseca nakon rekonstrukcije ACL-a prema brojnim istraživanjima je najznačajnije za provođenje programa vježbi za redukciju pojavnosti recidiva.
U radu je naveden detaljan opis rizičnih čimbenika, koji povećavaju postotak recidiva, te prikaz funkcionalne faze rehabilitacije koja će se koristiti za adaptaciju sportaša na te čimbenike i tako posljedično smanjiti njihov utjecaj na pojavnost reruptura. Ovim radom je opisan novi smjer utjecaja različitih oblika vježbi na postojeće rizične faktore i zabrinjavajuću stopu recidiva rupture ACL-a.
Abstract (english) The knee is the most complex angular-rotary joint that tolerates the greatest loads, and precisely because of this, it is prone to various injuries. Ruptures of the anterior cruciate ligament (ACL) account for 50 % of all knee injuries in athletes. The rehabilitation process of an ACL rupture doesn't have standardized protocols, but rather basic guidelines to guiding through a six to eighteen months recovery. On the other hand, a major health problem in sports is the increasingly frequent recurrence of ACL ruptures, which occur in the highest percentage in football, although the incidence is increasing in other sports as well. Recognizing risk factors such as neurological and biomechanical changes, psychological factors and fatigue is crucial in order to reduce the incidence. According to numerous studies, the period between the sixth and ninth months after ACL reconstruction is the most important for implementing an exercise program to reduce the incidence of recurrence.
The thesis provides a detailed description of risk factors that increase the percentage of relapses, as well as a presentation of a functional phase of rehabilitation that will be used to adapt athletes to these factors and thus reduce their impact on the incidence of rerupture. This thesis describes a new direction of influence of different forms of exercise on existing risk factors and the worrying rate of recurrence of ACL rupture.
