Abstract
Bolesti miokarda, ponajprije ishemijska srčana bolest, veliki su javnozdravstveni problem u svijetu i vodeći uzrok smrti u većini razvijenih zemalja svijeta. U posljednjih 30-ak godina broj smrtnih slučajeva od IHD u tim zemljama se smanjio, ali u mnogim dijelovima svijeta taj broj i dalje raste (Rusija i neke istočnoeuropske zemlje). Mogućnost da sustav kalpaina igra ulogu u ovim bolestima eksperimentalno je istražena u literaturi i neke studije su se fokusirale na efekte inhibicije kalpaina u poboljšanju disfunkcije miokarda. Kalpaini su skupina nelizosomskih cisteinskih proteaza koji se aktiviraju u prisutnosti kalcija. Nekontrolirana aktivnost kalpaina povezana je s patogenezom reperfuzijske ozljede miokarda, hipertrofije miokarda, srčanog zastoja i ishemije miokarda. Inhibicija kalpaina može znatno usporiti proces zatajivanja srca i reducirati veličinu infarkta nakon ishemije-reperfuzije. Pretjerana aktivacija kalpaina igra ulogu u oštećenju mitohondrija i oksidativnoj fosforilaciji za vrijeme IRI. Stoga inhibicija kalpaina predstavlja potencijalnu terapijsku strategiju u liječenju bolesti miokarda.
Abstract (english)
Miocardial diseases, primarily IHD, are a major public health problem worldwide and leading cause of death in the most developed countries. In the last thirty years the number of deaths from IHD has decreased, but in many parts of the world this number continues to grow (Russia and some Eastern European countries). The possibility that the calpain system could play a role in these deseases has been experimentally investigated in the literature and several studies have focused on the effects of calpain inhibitors in improving myocardial dysfunction. Calpains are a group of non-lysosomal calcium-activated cystein proteases. Uncontrolled activation of calpain has been found to be involved in the pathogenesis of IRI, cardiac hypertrophy, myocardial stunning and cardiac ischemia. Calpain inhibition significanty attenuate myocardial stunning and reduced infarct size after ischemia-reperfusion. Calpain inhibition therefore serves as a potential medical therapy for patients suffering of myocardial diseases.
