Abstract Uvod. Mehanizam nastanka boli žvačnih mišića u temporomandibularnim
poremećajima nije razjašnjen. Neupitna je uloga raznih neuronskih receptora u
mehanizmu boli, a među najvažnijima se smatraju P2X i P2Y purinoreceptori.
Ciljevi. Cilj istraživanja bio je utvrditi ulogu P2X3 i P2Y2 receptora u mehanizmima
orofacijalne boli odnosno ispitati utjecaj jednostrane maseterične upale na obostranu
bolnost maseteričnih mišića i na ekspresiju P2X3 i P2Y2 receptora u trigeminalnim
ganglijima, te utjecaj P2X3 i P2Y2 antagonista na obostranu bol.
Materijali i metode. Istraživanje je provedeno na 98 štakora soja Wistar. Obostrana
bol je izazvana jednostranim intramaseteričnim injiciranjem complete Freund's
adjuvanta. Četiri dana nakon injiciranja complete Freund's adjuvanta mjeren je
maseterični prag boli von Frey anesteziometrom obostrano. Po završetku mjerenja
praga boli izolirani su maseterični mišići potrebni za histopatološku analizu upalnih
promjena te trigeminalni gangliji iz kojih se kvantitativnom real-time PCR metodom
određivala ekspresija P2X3 i P2Y2 gena. Utjecaj P2X3 i P2Y2 receptora na bol
maseteričnog mišića je ispitan injiciranjem P2X3 i P2Y2 antagonista, A-317491 (6 μg i
60 μg u 50 μl) i suramina (0,5 mg i 5 mg u 100 μl) u ipsilateralni mišić četiri dana
nakon izazivanja upale u vremenskim točkama od 15, 30, 60 i 120 minuta.
Rezultati. Obostrano smanjenje praga boli utvrđeno je četiri dana nakon
administracije complete Freund's adjuvanta. Histopatološki je potvrđena upala u
ipsilateralnom maseteru. A-317491 je povećao obostrani prag boli u vremenskim
točkama od 15, 30 i 60 minuta, dok ga je suramin povećao u svim vremenskim
točkama. Povećana ekspresija P2X3 mRNA utvrđena je u ipsilateralnom
trigeminalnom gangliju, dok je povećana ekspresija P2Y2 mRNA utvrđena u
kontralateralnom trigeminalnom gangliju.
Zaključci. Jednostrana maseterična upala uzrokuje obostranu maseteričnu alodiniju.
P2X3 i P2Y2 receptori funkcionalno utječu na maseteričnu bolnost i sugeriraju
uključenost P2X3 i P2Y2 receptora ispoljenih na razini trigeminalnih ganglija u upalnim
stanjima masetera
Abstract (english) Objectives. To examine the role of P2X3 and P2Y2 in pain mecahnisms i.e. to
evaluate the influence of unilateral masseter inflammation on bilateral masseteric
allodynia (BMA) and P2X3 and P2Y2 receptors expression changes in trigeminal
ganglia (TRG), and the effect of intramasseteric antagonists administration on BMA.
Materials and Methods. BMA was induced by unilateral intramasseteric complete
Freund's adjuvant (CFA) injection in rats and four days after bilateral head withdrawal
treshold (BHWT) was measured. P2X3 and P2Y2 receptors engagement in nocifensive
behaviour was evaluated by administration of two doses of P2X3 antagonist A-317491
(6 and 60 μg/50 μl) and two doses of P2Y2 antagonist suramin (0,5 and 5 mg/100 μl)
into inflammed masseter four days post CFA injection. BHWT were measured 15, 30,
60 and 120 minutes post antagonist administration. Bilateral masseter muscles (MM)
and TRG were isolated from CFA injected rats and evaluated histopathologically and
using quantitative real-time PCR (qRT-PCR) analysis, respectively.
Results. A BHWT decrease was assessed after CFA injection (p≤0,001). Ipsilaterally
to CFA administration MM inflammation was established histopathologically. In
comparison to controls, both A-317491 doses increased BHWT at 15, 30 and 60
minutes timepoints. Both doses of suramin raised BHWT at all timepoints compared
to controls. qRT-PCR analysis demonstrated enhanced P2X3 expression in TRG
ipsilaterally to CFA administration and P2Y2 expression upregulation in contralateral
TRG.
Conclusion. Unilateral masseter inflammation induces BMA in rats. This study
provides evidence that P2X3 and P2Y2 receptors can functionally influence MM
allodynia and suggest that P2X3 and P2Y2 receptors expressed in TRG neurons are
involved in masseteric inflammatory pain conditions
