Abstract U ovom radu dan je kratak pregled o važnosti adekvatne zone pričvrsne gingive u očuvanju zdravlja parodontnih tkiva. Predstavljeni su stavovi literature vezani za važnost, veličinu i utjecaj pričvrsne gingive na očuvanje pričvrska i razvitak gingivnih recesija. Primjerice ako je zona pričvrsne gingive nedostatna, ali su održavanje oralne higijene i kontrola plaka uredni, tada širina pričvrsne gingive ima malu važnost u održavanju parodontnog zdravlja. No, ako je gingiva pod utjecajem čimbenika poput većih količina subgingivnog plaka, dubokih subgingivnih preparacija ili vlačnih sila frenuluma i mišića alveolarne mukoze, tada zona pričvrsne gingive od minimalno 2 milimetra pokazuje veću otpornost gubitku pričvrska. Prikazane su snovne indikacije za kirurški zahvat koje uključuju: usku zonu pričvrsne gingive koja otežava provođenje oralne higijene, predortodontsku pripremu pacijenta, subgingivni smještaj rubova preparacija, visoke inzercije mišića alveolarne mukoze, estetske razloge, prekrivanje gingivnih recesija i poboljšanje kvalitete mekih tkiva oko implantata. Danas se za proširenje zone pričvrsne gingive primarno koriste tehnike slobodnog epiteliziranog gingivnog transplantata i slobodnog vezivnotkivnog transplantata, kao i njihove varijacije poput tehnike tuneliranja i tehnike tobolca. Cijeljenje tkiva nakon operativnog zahvata dešava se kroz 3 faze pri čemu dolazi do integracije transplantata uz ljuštenje i ponovno stvaranje epitelnog sloja, uz konačno sazrijevanje (maturaciju) tkiva. Osim prirodnih autogenih transplantata čije je porijeklo najčešće s nepca, tržište dentalnih materijala u posljednjih 15-ak godina nudi i alternative ksenogenog porijekla čija se klinička primjena pokazala uspješnom.
Abstract (english) The aim of this paper is to provide a brief overview of the importance of an adequate zone of attached gingiva for the preservation of periodontal tissue health. Attitudes found in literature vary to a great extent regarding the importance, size and impact of the attached gingiva on the prevention of attachment loss and development of gingival recessions. For example, while an adequate oral hygiene is maintained the width of attached gingive plays a minor role in the preservance of periodontal tissue health. On the other hand, if that same gingiva is subject to an increased subgingival plaque accumulation, subgingival restoration margin placement or high frenal or mucosa muscle pull, then a zone of 2 or more millimeters shows greater levels of resistance to the development of gingival recessions and attachment loss. Basic indications for surgical procedures are shown, which include: a deficient zone of attached gingiva which prevents proper conduct of daily oral hygiene, pre-orthodontic surgical treatment, subgingival placement of prosthetic preparations, high muscle insertions of alveolar mucosa, aesthetic reasons, gingival recessions coverage and improving the quality of soft tissues around implants. Basic surgical techniques and their variations were introduced. Techniques for the use of epithelized gingival grafts and connective tissue grafts as well as their variations such as tunneling techniques and pouch techniques have been described together with post-operative tissue healing.
