Abstract
Bruksizam se definira kao noćno škripanje ili stiskanje zubima koje je praćeno oštećenjem prirodnih zubi i protetskih nadomjestaka, pojavom zvukova ili bolnih senzacija. Manifestacije bruksizma na žvačnom sustavu su brojne. Bruksizam uzrokuje trošenje zuba i protetskih nadomjestaka, ostavlja trajne posljedice na endodontski prostor i parodontna tkiva, uzrok je povišenog mišićnog tonusa, boli i hiperaktivnosti mišića žvakača, promjena u temporomandibularnom zglobu te se često povezuje s pojavom glavobolja. Terapija bruksizma je veoma kompleksna i može samo olakšati simptome ali ne i izliječiti osnovnu bolest. U terapiji noćnog bruksizma koristi se stabilizacijska tvrda akrilatna udlaga, a u terapiji dnevnog bruksizma učinkovit je biofeedback pristup. Kod odluke za protetsku rehabilitaciju važno je procijeniti postoji li potreba za podizanjem VDO. Kod izrade potpunih proteza treba imati na umu da one mogu uzrokovati pojačanu resorpciju alveolarnog grebena, a kod djelomičnih proteza se voditi kriterijem izrade po principima minimalne ekstenzije. Kod bruksista je preporučljivo koristiti solo krunice kada god to klinička situacija dozvoljava. Plemeniti metali te metal-keramički sustavi, napogodniji su materijali za upotrebu kod bruskista. Glede implanto protetske terapije u bruksista, danas prevladava mišljenje da bruksizam ne utječe na samu osteointegraciju implantata, već na protetsku suprastrukturu, povećavajući rizik od tehničkih komplikacija. Po završetku bilo kojeg oblika protetske rehabilitacije obavezno je nošenje noćne udlage.
Abstract (english)
Bruxism is defined as night grinding and clenching of teeth, which causes damage to natural teeth and prosthetic superstructures and also includes the appearance of sounds and painful sensations. Dental manifestations of bruxism are numerous. Bruxism causes tooth wear, leaving permanent effects on the endodontial and periodontal teeth. It is a cause of elevated muscular tone, pain and hyperactivity of masticatory muscles, changes in the temporomandibular joint and is often associated with the appearance of headaches. The bruxism therapy is very complex and can only ease the symptoms but does not cure the underlying disease. In the treatment of night bruxism, a stabilizing hard acrylate byte is applied to the upper teeth. Biofeedback approach is effective in daily bruxism therapy. When deciding on prosthetic rehabilitation it is important to evaluate whether there is a need to increase VDO. When making full prostheses, the doctor should have in mind that they can cause increased resorption of the alveolar ridge, and in partial prostheses, using the principle of minimal extensions. It is recommended to use single crowns whenever the clinical situation permits doing so. Noble metals and metal-ceramic systems are the choice material to use when dealing with bruxists. Today's opinion is that bruxism does not affect osteointegration of dental implants, but increases risk of technical complications on prosthetic suprastructures. After completion of any form of prosthetic rehabilitation, it is necessarily to wear a nich oclusal byte.
