Abstract
Aneurizme truncusa coeliacusa su rijedak entitet u kliničkoj praksi, incidencije 0,01% svih aneurizmi. Nekada je rizik od rupture bio puno veći, a danas iznosi 50%. Zahvaljujući modernoj slikovnoj dijagnostici, pogotovo upotrebom ultrazvuka i MSCT-a, često su slučajan klinički nalaz. Ateroskleroza je najčešći uzrok suvremenih aneurizmi truncusa coeliacusa. Iako je većina pacijenata asimptomatska, u simptomatskih dominiraju gastrointestinalni simptomi: bol u trbuhu, mučnina, povraćanje. Rana dijagnostika i intervencija su od ključne važnosti. Indikacije za operativni zahvat su sve simptomatske aneurizme, aneurizme koje se povećavaju za više od tri do četiri puta u odnosu na promjer arterije, te kalcificirane aneurizme veće od tri centimetra. Endovaskularno liječenje pogodno je za visokorizične pacijente bez jetrene patologije i patologije kolateralne cirkulacije.
Abstract (english)
Celiac trunk aneurysms are a rare entity in clinical practice, an incidence of 0.01% of all aneurysms. Sometimes the risk of rupture was much higher, and today it amounts to 50%. Thanks to modern imaging modalities, especially the use of ultrasound and MSCT, they are often a random clinical finding. Atherosclerosis is the most common cause of modern celiac trunk aneurysms. Although most patients are asymptomatic, in symptomatic patients are dominant gastrointestinal symptoms that include abdominal pain, nausea, vomiting. Early diagnosis and intervention are of crucial importance. Indications for surgery are all symptomatic aneurysms, aneurysms that increase by more than three to four times the diameter of the artery, and calcified aneurysms greater than three centimeters. Endovascular treatment is suitable for highrisk patients without liver pathology and pathology of collateral circulation.
