Abstract Diabetes mellitus je široko rasprostranjena kronična metabolička bolest koja se prvenstveno očituje povišenom razinom glukoze u krvi, što s vremenom dovodi do ozbiljnih oštećenja drugih organa, kao što su srce, krvne žile, oči, bubrezi i živci. Najučestaliji oblik DM je DM tip II, koji nastaje kada tkiva postanu rezistentna na inzulin ili organizam ne stvara dovoljno inzulina. DM tip I je kronično stanje u kojem gušterača uopće ne proizvodi inzulin, ili ga proizvodi u vrlo malim količinama. DM predstavlja jedan od vodećih svjetskih zdravstvenih problema. Ranom dijagnostikom, promjenom načina života i pravilnim liječenjem može se produljiti očekivano trajanje života te značajno poboljšati njegova kvaliteta.
Parodontitis je kronična oralna infekcija i smatra se jednom od najčešćih upalnih bolesti čovječanstva. Glavni uzrok bolesti je bakterijski biofilm smješten subgingivno, zajedno s izraženom osjetljivošću domaćina. Razvojem bolesti dolazi do gubitka epitelnog pričvrstka i resorpcije alveolarne kosti, a ako se ne liječi, može uzrokovati gubitak zuba. Parodontitis se jednostavno dijagnosticira i moguće ga je liječiti uz pravilnu oralnu higijenu i česte posjete doktoru dentalne medicine.
Uzajamno djelovanje između parodontitisa i DM dugo je vremena predmet istraživanja, te dokazi prikupljeni do sada pokazuju vrlo složenu vezu između ove dvije različite patologije. Epidemiološke studije i klinička ispitivanja pokazuju kako dijabetičari s loše kontroliranom razinom serumske glukoze u krvi češće obolijevaju od parodontitisa, u usporedbi s dobro kontroliranim dijabetesom i zdravom populacijom. U isto vrijeme, parodontitis utječe na održavanje glikemijske kontrole. Parodontna terapija značajno smanjuje razinu HbA1c.
Abstract (english) Diabetes mellitus is a widespread chronic metabolic disease characterized by hyperglycemia, which over time leads to serious damage to other organs such as heart, blood vessels, eyes, kidneys and nerves. The most frequent DM is the Type II, it results from insulin resistance or increased production of insulin in pancreas. DM Type I results from autoimmune destruction of pancreatic β-cells, leading to a complete loss of insulin secretion. DM is one of the major health problems worldwid. Early diagnostic, lifestyle changes and proper treatment can prolong life expectany and significantly improve the quality of life.
Periodontitis is a chronic oral infection and is considered to be one of the most common inflamatory diseases of humanity. The main cause is subgingival bacterial biofilm, along with pronounced host susceptibility. Periodontitis is characterized by gingival inflammation, loss of connective tssue attachment and alveolar bone resorption, and if untrated, causes tooth loss. Periodontitis is easily diagnosed and clinically controlled - with regular high-quality supportive treatment, clinical results can be maintained.
The interaction between periodontitis and DM has long been the subject of research and the results show a bidirectional relationship between those two chronic diseases. Epidemiological studies and clinical trials show that people with sub-optimally controlled diabetes suffer from increasd periodontal inflamation, in comparison with people with controlled diabetes and healthy population. In the same time, periodontitis affects on glycemic controle. Periodontal treatment results in a significant reduction in glycates haemoglobin levels.
