| Abstract | Karcinom prostate najčešći je maligni tumor u muškaraca, te se u Republici Hrvatskoj prema učestalosti nalazi na prvom mjestu, dok je u svijetu drugi najčešći karcinom sa 1,3 milijuna novooboljelih u 2018. godini. Incidencija raka prostate razlikuje se ovisno o zemljopisnoj lokaciji, rasi, ekonomskom statusu, zdravstvenoj zaštiti i informiranosti pučanstva. Zasada nije poznata etiologija karcinoma, ali postoje određeni čimbenici rizika povezani s njegovim nastankom koje svrstavamo u nepromjenjive i promjenjive. U nepromjenjive čimbenike ubraja se životna dob, etnička pripadnost, te pozitivna obiteljska anamneza, dok u promjenjive okoliš, životni stil i drugi potencijenlno rizični čimbenici. Karcinom prostate često je multifokalan, a patohistološki najzastupljeniji je adenokarcinom kod kojeg razlikujemo acinusni i duktalni tip. Karcinom prostate je sporoprogredirajuća bolest kod koje klinička slika ovisi o stupnju proširenosti bolesti. Za procjenu gradusa adenokarcinoma koristi se Gleasonov sustav, dok za procjenu proširenosti TNM klasifikacija. S ciljem smanjenja morbiditeta i mortaliteta od karcinoma prostate vrlo je bitna rana detekcija kako bi se povećala uspješnost liječenja, ali i poboljšala kvaliteta života pacijenta. U ranoj detekciji koristi se digitorektalni pregled, određivanje PSA vrijednosti i TRUS biopsija prostate. Odluku o odgovarajućem liječenju donosi multidisciplinarni tim nakon terapijske procjene i klasifikacije pacijenta prema riziku. Nakon terapijske procjene odlučuje se o daljnjem nastavku liječenja, a ono može biti aktivno čekanje, praćenje ili liječenje. Liječenje karcinoma prostate može se provesti kirurškim metodama, radioterapijom, hormonskom terapijom ili kombinacijom. |
| Abstract (english) | Prostate cancer is the most common malignant tumor in men, and in the Republic of Croatia it ranks first in frequency, while in the world is the second with 1.3 million new cases in 2018. The incidence of prostate cancer varies depending on geographical location, race, economic status, health care, and public information. The etiology of cancer is currently unknown, but there are certain risk factors associated with occurrence that we classify as unchanging and variable. Unchanging factors include age, ethnicity, and a positive family history, while changing include environment, lifestyle, and other potentially risky factors. Prostate cancer is often multifocal, and pathohistologically the most common is adenocarcinoma, in which we distinguish acinar and ductal types. Prostate cancer is a slow-progressing disease in which clinical picture depends on the extent of the disease. The Gleason system is used to estimate the degree of adenocarcinoma, while the TNM classification to estimate the extent of the adenocarcinoma. With a view to reduce the morbidity and mortality from prostate cancer, early detection is important in order to increase the success of treatment, and to improve the quality of life of the patient. Early detection uses digitorectal examination, determination of PSA values and TRUS prostate biopsy. The decision on appropriate treatment is made by a multidisciplinary team after therapeutic assessment and classification of the patient according to risk. About further treatment is decided after the therapeutic assessment, and can be active waiting, monitoring or treatment. Prostate cancer treatment include surgical methods, radiotherapy, hormone therapy, or a combination. |
