Abstract Porođaj predstavlja kraj trajanja trudnoće, čin kojim se dijete ili više djece uz pomoć trudova
istiskuju iz maternice. Porođaj se može podijeliti u četiri porođajna doba, a tijek i ishod
normalnog porođaja određeni su porođajnim objektom (fetus i placenta), porođajnim kanalom
i porođajnim snagama (trudovi). Što se tiče porođajne boli, maternica i vrat maternice
inervirani su simpatičkim i parasimpatičkim vlaknima autonomnog živčanog sustava. Mnogo
čimbenika utječe na intenzitet, trajanje, raspodjelu i kakvoću bola tijekom porođaja, a oni se
mijenjaju kako sam porođaj napreduje. S obzirom da su porođaj i porađanje, bolni te stresni i
za majku i za dijete, upravo su se anestezija i analgezija pokazali kao jedan od ključnih
faktora za smanjivanje tog stresa. Naročito se tu korisnom pokazala epiduralna analgezija,
vrsta neuroaksijalne regionalne anestezije koja omogućava anesteziranje samo određenog
dijela tijela. Prednosti ovakve anestezije su daleko brojnije od nedostataka, a zabilježene su i
rijetke komplikacije. Jedna od komplikacija može biti i potreba za instrumentalnim dovršenjem poroda, bilo carskim rezom ili uporabom vakuum ekstraktora.
Ovom istraživanju primarni cilj je bio utvrditi koliki je udio rodilja koje su primile epiduralnu
analgeziju, a porod im je dovršen carskim rezom ili vakuum ekstrakcijom te utječe li
epiduralna analgezija statistički značajno na način dovršenja poroda (vaginalno ili
instrumentalno).
Svi ciljevi istraživanja su ispunjeni te je i dokazana osnovna hipoteza. Porodi rodilja, koje
prime epiduralnu analgeziju, statistički značajno češće se dovršavaju instrumentalno, bilo
carskim rezom ili vakuum ekstrakcijom.
Abstract (english) Childbirth represents an end of a pregnancy, the act by which a child or children, with the
help of contractions, are expelled from the uterus. Childbirth can be divided into four stages,
and the course and outcome of vaginal delivery are determined by birth object (fetus and
placenta), the birth canal and the birth forces (labor). As for the birth pains, the uterus and
cervix are innervated by sympathetic and parasympathetic fibers of the autonomic nervous
system. Many factors affect the intensity, duration, distribution and quality of pain during the
birth, and they are changing as the childbirth progresses. Given that labor and giving birth,
are painful and stressful for both mother and child, one of many key factors of reducing this
stress are anesthesia and analgesia. Especially useful is the usage of epidural analgesia, type
of neuraxial regional anesthesia that anesthetizes only a certain part of the body. The
advantages of such anesthesia are far more numerous than disadvantages, and there are rare
complications. One of the complications can be the need for the completion of childbirth by methods of instrumental delivery, either by caesarean section or vacuum extraction.
Primary objective of this study was to determine what proportion of pregnant women who had
received epidural analgesia had to undergo caesarean section or vacuum extraction and if
epidural analgesia affects significantly the mode of completion of delivery (vaginal or
instrumental).
All research objectives have been met and the basic hypothesis has been proven. Childbirth of
parturients, who receive epidural analgesia, is more often completed instrumentally, either by
cesarean section or vacuum extraction .
