Abstract
Čest uzrok bolnog ramena su ozljede rotatorne manšete, a one mogu biti traumatske ili degenerativne etiologije. Traumatske ozljede su posljedica pada ili preopterećenja ruke teškim teretom te se mogu dogoditi kod pacijenata svih dobnih skupina. Degenerativne promjene su posljedica starenja i ponavljajućih mikrotrauma koje preko sindroma sraza i tendinitisa dovedu do parcijalnih i potpunih ruptura. Degenerativne promjene su češće i zahvaćaju dobnu skupinu iznad 50 godina. Dijagnoza ozljede rotatorne manšete se postavlja uz pomoć anamneze, kliničkog pregleda i slikovne dijagnostike. Ruptura može biti djelomične ili potpune debljine, a daljnje karakteristike se dodjeljuju koristeći brojne klasifikacije ozljeda. Uzimajući u obzir dob pacijenta i njegove ciljeve i želje kao i karakteristike rupture odabire se neoperativni ili operativni način liječenja. Pacijenta koji se liječi konzervativno je potrebno učestalo pratiti te ne odgađati kirurško liječenje ukoliko je ono potrebno zbog pogoršanja simptoma. Danas je najčešće korištena artroskopska rekonstrukcija tetiva rotatorne manšete.
Abstract (english)
A common cause of painful shoulder is rotator cuff injury, whose etiology is either traumatic or degenerative. Traumatic rotator cuff injuries can be caused by a fall on the arm or overload with heavy weight in any age group. Degenerative changes result from aging and repetitive microtrauma that cause impingement syndrome and tendinitis, which lead to partial and full-thickness tears. Degenerative rotator cuff injuries are more common than traumatic, and the affected age group is over 50 years old. The rotator cuff injury diagnosis is met through anamnesis, physical exam, and imaging. Rupture can be either partial or full-thickness and is further classified. The treatment of choice is based upon the patient's age, goals, and sole injury qualities. Conservative therapy requires frequent follow-up and a surgeon's prompt reaction if the onset of worsening symptoms is noticed. If needed, surgery should not be postponed. Nowadays, arthroscopic repair is the most commonly used.
