Abstract Cilj istraživanja: Utvrditi učestalost i karakteristike nemelanomskih kožnih karcinoma maksilofacijalne regije.
Ispitanici i metode: Retrospektivnom studijom su, uvidom u IBIS sustav, obuhvaćeni ispitanici stariji od 18 godina, kod kojih je na Klinici za Maksilofacijalnu kirurgiju KBC-a Rijeka, u razdoblju od 01.01.2021. do 31.12.2021., ekscidiran i PH analizom potvrđen nemelanomski karcinom kože maksilofacijalne regije. Iz navedenih PH dijagnoza su zabilježeni osnovni podaci o pacijentima (dob, spol, godište) te podaci o analiziranom karcinomu (veličina, lokalizacija, tip, subtip...) koji su statistički obrađeni i uspoređivani sa sličnim provedenim studijama u svijetu i RH.
Rezultati: U analiziranoj skupini pacijenata, utvrđeno je da muškarci i žene podjednako obolijevaju od kožnih nemelanomskih karcinoma( 51%:49%, u korist muškaraca). Kao najčešća dijagnoza, ističe se bazocelularni karcinom koji čini 2/3 svih slučajeva, dok manji dio čini dijagnoza planocelularnog i ostalih karcinoma (poput adneksalnih karcinoma). Incidencija navedenih dijagnoza počinje rasti između 50. i 60. godine života, bez obzira na spol te ne postoji statistički značajna razlika u pojavnosti pojedinog tipa karcinoma, obzirom na spol. Skupina pacijenata s dijagnozom SCC-a je starija od skupine s dijagnozom BCC-a (za prosječno 5.2 god.). BCC se češće pojavljuje na nosu (25%) s predominacijom agresivnih subtipova (infiltrativna varijanta), dok se SCC (in situ varijanta) češće pojavljuje na obrazu (25%) u odnosu na ostale anatomske podjedinice glave i vrata. Utvrđena je statistički značajna razlika u pojavnosti karcinoma na anatomskim lokalizacijama obzirom na spol (p= 0.022), pa tako muškarci imaju veći broj novotvorina na vratu, uhu i obrazu u odnosu na žene.
Zaključci: Nemelanomski kožni karcinomi su podjednako česti kod muškaraca i kod žena. Dijagnoza BCC-a je češća od dijagnoze SCC-a. Bazaliomi se češće pojavljuju na nosu, a spinaliomi na obrazu. Muškarci imaju veću incidenciju karcinoma na vratu, uhu i obrazu od žena.
Abstract (english) Objectives: To determine the frequency and characteristics of non-melanoma skin cancers of the maxillofacial region.
Patients and methods: The retrospective study was conducted, with an insight into the IBIS system, on a clinical sample of patients, over the age of 18 who were treated at the Clinic for Maxillofacial Surgery of the Clinical Hospital Centre Rijeka, in the period from January 1, 2021. to December 31, 2021. From the pathohistological diagnoses, basic data of patients (such as age, sex and date of birth) and data of the analyzed cancers (such as size, location, type, subtype etc.) were recorded and statistically processed. The results obtained were compared with similar studies in the world and the Republic of Croatia.
Results: In the analyzed group of patients, it was found that men and women alike suffer from non-melanoma skin cancers (51% : 49%, in favor of men). The most common diagnosis is basal cell carcinoma, which accounts for 2/3 of all cases, while a smaller part is the diagnosis of squamous cell and other cancers (such as adnexal carcinomas). The incidence of these diagnoses begins to increase between 50 and 60 years of age, regardless of gender and there is no statistically significant difference in the incidence of each type of cancer depending on gender. The group of patients dignosed with SCC is older than the group diagnosed with BCC (by average of 5.2 years). BCC occurs more frequently on the nose (25% of cases) with a predominance of aggressive subtypes (infiltrative variant), while SCC (in situ variant) occurs more frequently on the cheek (25%) compared to other anatomical subunits of the head and neck. There was statistically significant difference in the incidence of cancer at certain anatomical sites according to gender (p= 0.022), so men have a higher incidence of tumours on the neck, ear and cheek compared to women.
Conclusions: NMSC are equally common in men and women. A diagnosis of BCC is more common than a diagnosis of SCC. Basaliomas are more common on the nose and spinaliomas on the cheeks. Men have a higher incidence of cancer of the neck, ear and cheek than women.
