Abstract Cilj istraživanja bio je ispitati odnose između izraženosti dentofacijalnog deformiteta,
narušenosti kvalitete života i crta ličnosti. Dalje, cilj je bio utvrditi motive za prihvaćanje
ortognatske kirurgije i ispitati njezin učinak na kvalitetu života te validirati Upitnik
ortognatske kvalitete života (OQLQ) koji mjeri funkcijske i psihosocijalne efekte
skeletne malokluzije u hrvatskom kulturnom okruženju.
Uzorak su činila 472 ispitanika (65 % ženskog spola) u dobi od 12 do 45 godina.
Stupanj malokluzije procijenjen je Indeksom potrebe za ortognatskom funkcionalnom
terapijom. Procijenjene su crte ličnosti ekstraverzija, neuroticizam, ugodnost,
otvorenost, savjesnost, perfekcionizam i samopoštovanje te dimenzije OQLQ.
Validacija OQLQ upitnika obuhvatila je provjeru valjanosti, prikladnosti i pouzdanosti.
Regresijskom analizom istražen je odnos izraženosti dentofacijalnog deformiteta,
narušenosti kvalitete života, prihvaćanje predloženog kirurškog zahvata i odnos s
crtama ličnosti.
Hrvatska inačica OQLQ upitnika pokazala je psihometrijska svojstva slična izvorniku.
Stupanj izraženosti dentofacijalnog deformiteta utječe na samoprocijenjenu
narušenost kvalitete života. Ispitanike koji su prihvatili predloženi kirurški zahvat
karakterizirala je starija dob, perfekcionizam, zabrinutost zbog estetike lica, socijalni
aspekt i narušenost oralne funkcije te niže samopouzdanje s malom do srednjom
veličinom učinka (p ≤ 0,040; r = 0,198 – 0,399). Ostale osobine ličnosti i svijest o
dentofacijalnom deformitetu nisu se razlikovale među skupinama. Multipla logistička
regresija uz kontrolu spola, stupnja perfekcionizma i samopoštovanja pokazala je da
su izgledi za prihvaćanje predložene ortognatske kirurgije 3,4 puta veći kod osoba s
visokom zabrinutosti zbog estetike lica (omjer izgleda (OR) = 3,4; 95 % Interval
pouzdanosti (CI) = 1,3-9,1), 3 puta veći kod onih s narušenom funkcijom (OR = 3,0;
95% CI 1,0 - 8,6) i 2,7 puta veći u osoba starijih od 18 godina (OR = 2,7; 95 % CI 1,0
-7,1; p<0,001). Ortognatska kirurgija ima najnaglašeniji pozitivan učinak na društvene
kontakte i zabrinutost zbog estetike lica, a najmanji na svijest o dentofacijalnom
deformitetu.
Abstract (english) The aim of the study was to explore the relationship between intensity of dentofacial
deformity, impairment of quality of life and personality traits. The motives for
acceptance of conventional orthodontic-surgical treatment and its effect on quality of
life were also explored. The evaluation of psychometric properties of Orthognathic
Quality of Life Questionnaire (OQLQ) that measures functional and psychosocial effect
of skeletal malocclusion was also performed in the Croatian cultural context.
The sample consisted of 472 subjects (65% female) aged 12 to 45 years. The intensity
od skeletal malocclusion was evaluated with the Index of Orthognathic Functional
Treatment Need. The personality traits extraversion, neuroticism, agreeableness,
openness, consciousness, perfectionism and self-esteem, and dimensions of quality
of life were explored. The psychometric properties of the OQLQ were assessed by
checking validity, reliability, and responsiveness. Regression analysis was used to
explore relationship between intensity of dentofacial deformity, quality of life
impairment and personality traits.
The Croatian version of the OQLQ psychometric properties proved to be similar to the
original version. The intensity of skeletal malocclusion affects subject's quality of life.
The subjects who accepted suggested surgical procedure had higher age,
perfectionism, facial aesthetic concern (FA), social aspect (SA) and impairment of oral
function (OF) and lower self-esteem, with small to medium effect size (p≤0.040;
r=0,198-0,399). Other personality traits and awareness of dentofacial deformity (AW)
did not differ between the groups. Multiple logistic regression analysis shows that,
while controlling for gender, perfectionism and self-esteem, odds for accepting
orthognathic surgery were 3,4 times higher in subjects highly concerned with facial
aesthetics, FA (Odds ratio (OR) = 3,4; 95% Confidence interval (CI)=1,3-9,1), 3 times
in subjects with compromised oral function, OF (OR=3,0; 95% CI 1,0-8,6) and 2,7 times
in those older than 18 years (OR=2.7; 95% CI 1,0-7,1; p<0.001).
The most pronounced positive effect of orthognathic surgery was on social aspect and
aesthetic concern, and least on awareness of dentofacial deformity.
