Abstract Kada govorimo o željezu spominjemo ga kao jednog od glavnih biogenih metala. Željezo je neophodno za prijenos kisika u organizmu te je sastavni dio mnogih enzima i vitamina. Uglavnom se u medicini navodi problem manjka željeza zbog njegove spore apsorpcije u probavnom traktu, a čestih gubitaka, uglavnom kod ženske populacije. Za razliku od toga u srednje teškim i teškim oblicima COVID-19 infekcije, zabilježene su izrazito visoke razine ovog metala u cirkulaciji. Istraživanja stanja visoke razine željeza u krvi otkrile su zanimljiva saznanja. Dokazi upućuju na to da SARS-CoV-2 virus, uzročnik COVID-19 bolesti ima afinitet prema eritrocitima, točnije 1-beta lancima hemoglobina. Zbog virusa dolazi do raspada ovih jedinica pri čemu se oslobađa željezo u ionskom obliku koji završava slobodan u stanici ili u cirkulaciji i uzrokuje niz zdravstvenih komplikacija. Najznačajnija od njih je direktno djelovanje željeza putem Haber-Weiss-ove reakcije točnije, njenog drugog stupnja fenton reakcije. Posredstvom te reakcije nastaju spojevi reaktivne kisikove vrste (ROS). Ovi nestabilni spojevi uzrok su oksidativnog stresa koje oštećuje stanične komponente. Posljedično tome nastaje niz reakcija koje rezultiraju pojačanjem upale i uz prisutnu hipoksiju obično dovode do multiorganskog oštećenja. Kada se tome pridoda direktno patološko i inflamatorno djelovanje respiratorne infekcije SARS-CoV-2 virusa, stanje može progredirati i u organsko zatajenje ili čak smrti. Zahvaljujući ovim saznanjima predviđa se kako bi kelatori željeza (spojevi koji na sebe vežu željezove ione i neke od spojeva željeza, ali i neke druge metale) mogli imati značajni terapijski učinak u liječenju srednje teških i teških oblika COVID-19 infekcije.
Abstract (english) When we talk about iron, we mention it as one of the main biogenic metals. It is essential for the transfer of oxygen in the body and an integral part of many enzymes and vitamins. The issue frequently mentioned in medicine is the one of iron deficiency that occours due to its slow absorption in the digestive tract and frequent losses, mainly in the female population. In contrast, extremely high levels of this metal in circulation have been noted in moderate and severe forms of COVID-19 infection. Studies of the conditions of high iron levels in the blood have revealed interesting findings. Evidence suggests that the SARS-CoV-2 virus, the causative agent of COVID-19 disease, has an affinity for erythrocytes, namely 1-beta chains of hemoglobin. The virus causes a breakdown of these units, thus releasing iron in an ion form, which becomes free in the cell or in circulation and causes a number of health complications. The most significant of these is the direct effect of iron through Haber-Weiss's reaction, more precisely, its second-degree fenton reaction. Through this reaction, reactive oxygen species (ROS) are formed. These unstable compounds are the cause of oxidative stress that damages cellular components. Consequently, a series of reactions occur that result in increased inflammation and with the presence of hypoxia usually lead to multiorgan damage. Combined with the direct pathological and inflammatory effect of SARS-CoV-2 virus respiratory infection, the condition can progress to organe failure or even death. It is predicted that iron chelators (compounds that bind iron ions and some of the iron compounds to themselves, as well as some other metals) could have a significant therapeutic effect in the treatment of moderate and severe forms of COVID-19 infection.
