Abstract Tinitus ili šum u uhu jedan je od najranije opisivanih medicinskih pojava. Definira se kao svjesna percepcija zvuka koja se pojavljuje u odsutnosti vanjskog izvora podražaja koju pojedinac može osjećati unutar uha ili glave. Najčešće se šum opisuje kao zvonjava, šištanje i cvrčanje iako postoje i slučajevi gdje opisan kao složeniji poput melodije ili glasova. Glavna podjela tinitusa je na dvije skupine, a to su objektivni i subjektivni tinitus. Tinitus nije bolest, već simptom raznih patoloških stanja te stoga zahtjeva rano prepoznavanje i dijagnostičku obradu. Procijenjeno je da šum u uhu pogađa oko 15% svjetske populacije, od čega u 20% signifikantno utječe na kvalitetu života. Neugodan šum u uhu ometa pojedinca u dnevnim aktivnostima, te je njegov utjecaj na kvalitetu života uočen u okviru povećane pojavnosti depresije, anksioznosti, somatskih simptoma, poremećaja kvalitete sna, smanjenja koncentracije i pamćenja, poteškoća u razumijevanju govora i komunikaciji te problema na poslu. Pri procjeni potencijalnog uzroka ovog neugodnog simptoma, potrebno je u obzir uzeti heterogenu prirodu tinitusa te kod pristupa pacijentu sa šumom imati na umu brojna stanja koja ga uzrokuju kako bi se izabrala što primjerenija terapija. Iako postoje brojni terapijski pristupi tinitusu, potpuni nestanak simptoma najčešće nije moguć. Dostupni oblici terapije olakšavaju pacijentima da se suoče sa negativnim posljedicama, priviknu na šum u uhu te da ga djelomično ili potpuno maskiraju.
Abstract (english) Tinnitus or noise in the ear is one of the earliest described medical phenomena. It is defined as the conscious perception of sound that occurs in the absence of an external source of stimuli that an individual can feel inside the ear or head. Most often, the noise is described as ringing, hissing and sizzling, although there are also cases where it is described as more complex, such as a melody or voices. The main division of tinnitus is into two groups, objective and subjective tinnitus. Tinnitus is not a disease, but a symptom of various pathological conditions and therefore requires early recognition and diagnostic treatment. It is estimated that tinnitus affects about 15% of the world's population, of which 20% significantly affects the quality of life. Unpleasant ringing in the ear interferes with the individual's daily activities, and its impact on the quality of life has been observed in the context of increased incidence of depression, anxiety, somatic symptoms, sleep quality disorders, decreased concentration and memory, difficulties in understanding speech and communication, and problems at work. When assessing the potential cause of this unpleasant symptom, it is necessary to take into account the heterogeneous nature of tinnitus and, when approaching a patient with tinnitus, to keep in mind the numerous conditions that cause it in order to choose the most appropriate therapy. Although there are numerous therapeutic approaches to tinnitus, the complete disappearance of symptoms is usually not possible. The available forms of therapy make it easier for patients to face the negative consequences, get used to tinnitus and mask it partially or completely.
