Abstract
Anestezija je neizostavna sastavnica u kirurškom liječenju, a jedinstveni izazov u pripremi i odabiru pristupa predstavljaju djeca zbog svojih anatomskih, fizioloških i psiholoških osobitosti. Kad tome još nadodamo astmu kao kroničnu bolest dišnih puteva obilježenu opstrukcijom, upalom i hiperreaktivnosti dišnog puta, priprema i izbor anestezije su još važniji i izazovniji kako bismo spriječili razvoj komplikacija. Prvi korak k uspješnom liječenju je postavljanje kliničke dijagnoze i detaljan fizikalni pregled kako bi se ustanovio stupanj astme u djeteta. Potrebno je optimizirati prijeoperacijsko liječenje, ovisno o težini klasifikacije kronične astme, simptomima te lijekovima pomoću kojih u svakodnevnom životu kontroliraju astmu. Dijete s dobro kontroliranom astmom i minimalnim simptomima može nastaviti s anestezijom. Kada god je moguće, prvi izbor bi bila regionalna anestezija, a najsigurnija sredstva za indukciju opće anestezije su propofol, halotan i sevofluran. Ukoliko je moguće, intubacija se treba izbjegavati. Najčešća i za život opasna komplikacija je bronhospazam, no s današnjim medicinskim znanjem i tehnologijom komplikacije se mogu predvidjeti, prepoznati i pravovremeno liječiti.
Abstract (english)
Anesthesia is an indispensable component in surgical treatment, and children present a unique challenge in the preparation and selection of the approach due to their anatomical, physiological and psychological peculiarities. In order to prevent the development of complications, the preparation and choice of anesthesia are even more important and challenging with the addition of asthma as a chronic disease of the respiratory tract characterized by obstruction, inflammation and hyperreactivity of the respiratory tract. The first step towards a successful treatment is establishing a clinical diagnosis and a detailed physical examination to determine the degree of asthma in the child. It is necessary to optimize the preoperative treatment depending on the severity of the classification of chronic asthma, the symptoms and the drugs used to control asthma in everyday life. A child with well-controlled asthma and minimal symptoms can continue with anesthesia. However, the first choice should be regional anesthesia whenever possible. The safest means for induction of general anesthesia are propofol, halothane and sevoflurane. If possible, intubation should be avoided. The most common and life-threatening complication is bronchospasm, but with today's medical knowledge and technology, complications can be predicted, recognized and treated in a timely manner.
