Abstract Većina djece koja oboli od infekcije koronavirusne bolesti 2019 (COVID-19) u potpunosti će se oporaviti, dok određen broj pedijatrijske populacije može razviti dugoročne posljedice virusa na različitim organskim sustavima. Post-COVID-19 sindrom definira se kao stanje koje obično nastaje nakon 3 mjeseca od početka vjerojatne ili potvrđene SARS-CoV-2 infekcije, a simptomi perzistiraju najmanje 2 mjeseca i ne mogu se objasniti drugim stanjima. Smatra se da je u patofiziologiji sindroma izravno djelovanje virusa, oksidativni stres, imunološke abnormalnosti te pretjerani upalni odgovor. Češće je pogođen ženski spol i adolescenti, djeca koje nisu procijepljena protiv COVID-19, djeca nižeg socioekonomskog statusa te djeca s kroničnim bolestima u anamnezi, poput astme i šećerne bolesti. Kliničke manifestacije sindroma vrlo su širokog spektra i uključuju od općih simptoma do simptoma specifičnih za pojedine organske sustave. Najčešće prijavljeni simptomi kod djece jesu kronični umor, glavobolja, dispneja, gubitak apetita, anosmija, tjeskoba, gubitak tjeselne težine i mnogi drugi. Smjernice za dijagnozu u djece još uvijek nisu dostupne, a zbog nespecifičnih simptoma, dijagnoza se temelji na isključivanju drugih stanja. Predlaže se da sva djeca s infekcijom SARS-CoV-2 budu pregledana od strane specijalista pedijatrije 4. i 12. tjedana nakon infekcije kako bi se utvrdilo postojanje trajnih simptoma koje bi upućivale na post-COVID sindrom. Post-COVID je samolimitirajuća bolest te se terapija najčešće svodi na simptomatsko liječenje. Većina djece se oporavi bez značajnih zdravstvenih tegoba i bez primjene farmakoterapije. Ovisno o kliničkoj slici mogu se razmotriti uobičajeni lijekovi za specifične simptome.
Abstract (english) Most children who get infected with coronavirus disease 2019 (COVID-19) are expected to fully recover, while a certain percentage of children may develop long-term symptoms on various organ systems. Post-COVID-19 syndrome is characterized as a condition that emerges in individuals with a history of probable or confirmed SARS-CoV-2 infection, usually 3 months after the initial onset of infection with symptoms lasting for at least 2 months, which cannot be attributed to other underlying conditions. The pathophysiology of the syndrome is most likely a result of direct effect of the virus, oxidative stress, immune abnormalities and hyperinflammation. Females and adolescents, unvaccinated children against COVID-19, children from lower socioeconomic backgrounds, and those with a history of chronic illnesses are more commonly affected. The clinical manifestations of the syndrome are extensive and include general symptoms to symptoms associated with specific organ systems. The most frequently reported symptoms in children are fatigue, headache, dyspnea, loss of appetite, anosmia, body weight loss and many others. Guidelines for the diagnosing are currently unavailable, and due to nonspecific symptoms, the diagnosis is based on the exclusion of other conditions. It is suggested that all children with SARS-CoV-2 infection are screened by a pediatric specialist at 4th and 12th week post-infection to determine the presence of persistent symptoms that may indicate the presence of post-COVID syndrome. Post-COVID is a self-limiting disease and treatment is usually limited to symptomatic treatment. Most children recover without significant health problems and do not require medications. Depending on the clinical condition, medications for specific symptoms may be considered.
