Abstract
Podaci iz najnovijeg popisa stanovništva pokazuju da se Repubilka Hrvatska suočava sa problemom demografskog starenja populacije. Palijatvina skrb predstavlja jedan od važnijih strateških ciljeva u zdravstvenoj skrbi u Republici Hrvatskoj. Organizacija palijativne skrbi određena je Strateškim planom razvoja palijativne skrbi za razdoblje 2014.-2016. godine. Specifičnost ovakvog oblika skrbi je usmjerenost na bolesnika i interdisciplinarni pristup. Takav pristup podrazumijeva suradnju većeg broj djelatnika različitih profesija ovisno o potrebama bolesnika. Također je specifično da bolesnik cijelo vrijeme svog liječenja kruži unutar sustava i nema tipičnog otpusta bolesnika već se radi o preuzimanju bolesnika i obaveznom kontinitetu skrbi. Radi osiguravanja kvalitete palijativne skrbi i osiguravanja bržeg i jednostavnijeg obavljanja zadataka na korist palijativnog tima i samog bolesnika i obitelji, potrebno je osigurati dobre komunikacijske putove. Prije svega to podrazumijeva uvođenje normi i jedinstvene dokumentacije te izradu jasnih komunikacijskih putova kako ne bi dolazilo do gubljenja u sustavu. Napretkom informatizacije moguće je olakšati i ubrzati komunikaciju unutar palijativnog tima uvođenjem elektroničkog zdravstvenog zapisa palijativnog bolesnika i integeriranog informatičkog sustava palijativne skrbi koji bi povezivao sve članove tima. Cilj ovog rada bio je prikazati važnost komunikacije između članova palijativnog tima i ispravan tok informacija unutar tima. Prikazati koje informacije je potrebno razmjenjivati i na koji način dobra komunikacija može pridonijeti poboljšanju kvalitete palijativne skrbi.
Abstract (english)
Data from the latest Census show that Croatia faces the problem of demographic ageing. Palliative care is an important strategic goal in Crotian health care system. Organising of palliative care is outlined in the Strategic Plan for Palliative Care Development 2014 – 2016. This type of care is patient – oriented and interdisciplinary which means that a number of various professionals work together, depending on the patient's needs. The patient is never formally discharged from the system; instead he/she moves through it from one institution to another which ensures continuous care. To provide quality and promptness of palliative care and to simplify tasks as much as possible for the patient, family members and the palliative team, good communication paths must be established. This entails introduction of norms and uniform documentation and establishing clear communication paths to avoid losses in the system. Fast development of information technology facilitates and accelerates communication within a palliative team by introducing electronic health care records of palliative patients and an integrated IT system of palliative care which connects all members of the team. By showing which information must be exchanged and how good communication contributes to the quality of palliative care.
