Abstract
Sindrom iritabilnog crijeva (SIC) heterogeni je poremećaj koji se manifestira abdominalnim bolovima/nelagodom i promjenama u pražnjenju stolice u obliku konstipacije, proljeva ili kombinacije navedenih. Spada u najčešće probavne poremećaje i čest je problem u primarnoj i sekundarnoj zdravstvenoj zaštiti. Iako ne predstavlja životno ugrožavajuće stanje, SIC znatno narušava kvalitetu života i financijski opterećuje zdravstveni sustav. Posljedica je raznih etioloških mehanizama poput poremećaja gastrointestinalnog motiliteta, visceralne preosjetljivosti, te psihosocijalnih i genskih čimbenika. Nije pronaĎen prikladan dijagnostički biljeg već se dijagnoza sindroma postavlja na temelju kliničke slike i isključenja organske bolesti. Ustanovljeni su jasni dijagnostički kriteriji i algoritmi za početnu procjenu bolesnika koji se prezentira sa simptomima SIC-a. Unatoč pretpostavljenim raznim etiološkim mehanizmima nastanka bolesti, liječenje je ograničeno na ublažavanje simptoma i prevenciju rizičnih čimbenika. Ne postoji jedan lijek koji bi bio pogodan za sve bolesnike, ali dostupne su razne nefarmakološke i farmakološke metode liječenja. Temelj terapije i dalje bi trebao biti dobar odnos liječnika i bolesnika.
Abstract (english)
Irritable bowel syndrome (IBS) is a heterogenous disorder manifested by abdominal pain/discomfort and changes in bowel habits in the form of constipation, diarrhea or both. It is one of the most common gastrointestinal disorders and frequent problem in primary and secondary care. Although not life-threatening, IBS reduces patients’ quality of life and imposes a significant economic burden to the healthcare system. Various etiological mechanisms have been proposed as a cause, such as disorder of gastrointestinal motility, visceral hypersensitivity, and psyhosocial and genetic factors. Appropriate diagnostic marker has yet to be found, and diagnosis of syndrome is based on clinical presentation and exclusion of organic disease. Clear diagnostic criteria and algorithms have been established for patients presenting with IBS symptoms. Despite proposed various etiological mechanisms of disorder, treatment is limited to symptom relief and prevention of risk factors. A drug suitable for all patients has not been found, but different non-pharmacological and pharmacological methods are available. Basis of therapy should still be solid physician-patient relationship.
