Abstract
Sindrom policističnih jajnika(PCOS) najčešći je endokrinološki poremećaj žena reprodukcijske dobi i uzrok je 80% anovulacija. Smisao indukcije ovulacije kod pacijentica s PCOS je postići monoovulaciju radi izbjegavanja rizika nastanka ovarijskog hiperstimulacijskog sindroma i višeplodnih trudnoća. U pretilih žena s PCOS kao prva linija liječenja preporuča se promjena životnih navika i redukcija tjelesne težine primarno radi optimizacije općeg zdravstvenog stanja. Barijatrijska kirurgija kao terapijska opcija dolazi u obzir kod žena indeksa tjelesne mase ≥35 kg/m2 kod kojih terapija dijetom i tjelovježbom nije polučila očekivane rezultate. U prvoj liniji farmakološke indukcije ovulacije kod pacijentica s PCOS stoji uporaba klomifen citrata i letrozola. Uporaba metformina samog ima ograničenu dobrobit u smislu povećanja stope živorođene djece. U drugoj liniji liječenja anovulacija kod pacijentica s PCOS, kod kojih primjena klomifena i letrozola nije rezultirala trudnoćom, stoji korištenje gonadotropina i laparoskopska ovarijska dijatermija. Nema jasnih dokaza o djelotvornosti akupunkture i dodataka prehrani za indukciju ovulacije kod žena s PCOS. Kao zadnja linija liječenja anovulacija uzrokovanih PCOS na raspolaganju stoji in vitro fertilizacija(IVF). Ova se metoda pomognute oplodnje koristi i kod žena koje imaju uz PCOS i druge indikacije za njeno korištenje. Pri provođenju IVF kod pacijentica s PCOS prednost imaju protokoli koji koriste antagoniste GnRH. Metformin se može dodati navedenom protokolu sa svrhom izbjegavanja ovarijskog hiperstimulacijskog sindroma. Žene s PCOS imaju povećan rizik nastanka čitavog niza komplikacija u trudnoći, stoga čak i jednoplodna trudnoća kod žena s PCOS predstavlja rizik i za majku i za dijete.
Abstract (english)
Polycystic ovary syndrome (PCOS) is the most common endocrinopathy of women of reproductive age and the cause of anovulatory infertily in 80% of cases. The goal of the induction of ovulation in PCOS patients is achieving monofollicular anovulation in order to avoid the risk for ovarian hyperstimulation syndrome and multiple pregnancies. Lifestyle intervention is recommended as first line therapy in obese women with PCOS on the basis of general health benefits. Bariatric surgery can be considered in women with BMI ≥35 kg/m2 in whom lifestyle interventions has failed. Ovulation induction with chlomiphene citrate and letrozol stands in first-line pharmacological anovulation therapy. Metformin alone has limited benefits in improving live birth rates. In patients with PCOS who failed to ovulate or conceive with clomiphene and letrozol, gonadotropins and laparoscopic surgery can be used as second-line treatment. There is no clear evidence for efficacy of acupuncture or nutrient supplements in women with PCOS. For women with PCOS who fail lifestyle and ovulation induction therapy or have additional infertility factors, IVF can be used with the safer GnRH antagonist protocol. In IVF protocols addition of metformin may reduce the risk of ovarian hyperstimulation syndrome. Women with PCOS have an increased risk of developing a number of complications in pregnancy, so even singleton pregnancy in women with PCOS represents a risk for both mother and child.
