Abstract
Zbog sve veće učestalosti koksartroze ugradnja totalne endoproteze kuka postala je jedna od najčešće izvođenih ortopedskih operacija. Tom operacijom se bolesnicima uklanjaju simptomi koji su im ograničavali kvalitetu života. Danas se postoperativni ishod nakon ugradnje totalne endoproteze kuka učinjen zbog koksartroze mjeri upitnicima koje bolesnici sami ispunjavaju (engl. Patient Reported Outcome Measures (PROM)). Postoje generički upitnici kojima se procjenjuje opće zdravlje i upitnici specifični za degenerativnu bolest kuka. Najčešće korišteni generički upitnici su Kratki oblik upitnika o zdravlju (engl. Short Form Health Survey (SF)) koji može imati 36, 20, 12 i 8 pitanja te Europski upitnik o 5 dimenzija kvalitete života (engl. European Quality of Life-5 Dimensions (EQ-5D)). Od upitnika koji su specifični za procjenu stanja degenerativne bolesti kuka najčešće korišteni su Harrisov upitnik za kuk (engl. Harris Hip Score (HHS)), Indeks osteoartritisa Western Ontario McMaster sveučilišta (engl. Western Ontario and McMaster Universities Osteoarthritis index (WOMAC)), modificirani Merle d'Aubigne i Postelov upitnik (engl. modified Merle d'Aubigne and Postel Score (MDA)), Oxfordski upitnik za kuk (engl. Oxford Hip Score (OHS)), Upitnik za stanje nesposobnosti i osteoartritisa kuka (engl. Hip disability and Osteoarthritis Outcome Score (HOOS)), Upitnik za kuk i koljeno Američke akademije ortopedskih kirurga (engl. American Academy of Orthopaedic Surgeons' Hip and Knee Score (AAOSHKS)), Funkcionalna skala donjih ekstremiteta (engl. Lower Extremity Functional Scale (LEFS)), Charnleyev upitnik za kuk (engl. Charnley Hip Score (CHS)), Larsonov upitnik, te Lequesneov indeks težine osteoartritisa kuka (engl. Lequesne Index of Severity for Osteoarthritis of the Hip (LISOH)). Cilj ovog diplomskog rada je pregledno prikazati PROMs upitnike koji se najčešće koriste u literaturi za samoprocjenu ishoda ugradnje totalne endoproteze kuka. Od svih navedenih upitnika specifičnih za koksartrozu OHS ima najbolje kvalitete PROMs upitnika.
Abstract (english)
Due to the increasing prevalence of coxarthrosis, total hip arthroplasty has become one of the most frequently performed orthopedic operations. This surgery eliminates the symptoms that limit the patient's quality of life. Postoperative quality of life after total hip arthroplasty is assessed by the PROM (Patient reported outcome measures) questionnaires. There are generic PROM questionnaires for assessing general health and disease-specific questionnaires. The most frequently used generic questionnaires are Short Form Health Survey (SF) with 36, 20, 12 or 8 questions and European Quality of Life-5 Dimensions (EQ-5D). Most frequently used disease-specific questionnaires are Harris Hip Score (HHS), Western Ontario and McMaster Universities Osteoarthritis index (WOMAC), Hip disability and Osteoarthritis Outcome Score (HOOS), Oxford Hip Score (OHS), Larson's questionnaire, modified Merle d'Aubigne and Postel Score (MDA), Charnley Hip Score, Lower Extremity Functional Scale (LEFS) American Academy of Orthopaedic Surgeons' Hip and Knee Score (AAOSHKS) and Lequesne Index of Severity for Osteoarthritis of the Hip (LISOH). The aim of this graduate thesis is to present the PROMs questionnaires that are most frequently used in the self-assessment of the total hip endoprosthesis outcomes. PROMs questionnaires serve as appropriate instrument to evaluate the outcome of the treatment as patient's personal satisfaction with postoperative quality of life is the most important indicator of the success of the treatment. OHS proved to be the highest quality disease-specific questionnaire.
