Abstract
Robotska kirurgija se koristi u medicini već tridesetak godina. U zadnjih deset godina raste korištenje robotske kirurgije u liječenju benignih i malignih tumora lokaliziranih području glave i vrata. Dosad se pokazala korisnom u liječenju tumora orofarinksa, hipofarinska, larinksa, štitnjače, doštitnih žlijezdi, metastaza u limfnim čvorovima vrata te karcinoma nepoznatog primarnog podrijetla. Najčešći način korištenja je transoralni pristup orofarinksu, hipofarinsku i larinksu, čime se ostvaruje minimalna invazivnost uz prednost poboljšane trodimenzionalne vizualizacije i lakšeg manevriranja instrumentima, čime se mogu ostvariti bolji funkcionalni ishodi u odnosu na druge načine liječenja. Za potrebe liječenja tumora štitnjače, doštitnih žlijezdi i metastaza u limfnim čvorovima vrata razvijeno je nekoliko pristupa kojima je glavna prednost, u odnosu na otvoreni zahvat, znatno bolji kozmetski ishod. U slučaju karcinoma nepoznatog primarnog podrijetla robotskom kirurgijom je moguće odrediti primarnu lokalizaciju u većeg broja pacijenata. Glavni nedostatak robotske kirurgije je visoka cijena nabave i održavanja sustava, zbog čega se koristi u malom broju specijaliziranih centara. Bez obzira na nedostatke, radi se o vrijednoj metodi liječenja koja u ovom trenutku ima svoja ograničenja. Napretkom tehnologije i sve većim korištenjem robotske kirurgije u liječenju tumora glave i vrata očekuje se kako će se trenutni nedostatci umanjiti i upotreba robotske kirurgije proširiti.
Abstract (english)
Robotic surgery has been used in medicine for about thirty years. Usage of robotic surgery in the treatment of both benign and malignant tumors localized in head and neck has been rising for the last ten years. So far it has been found useful in the treatment of tumors of the oropharynx, hypopharynx, larynx, thyroid gland, parathyroid glands, neck lymph node metastases and cancer of unknown primary localization. It is most commonly used in a transoral approach to oropharynx, hypopharynx, and larynx, where its superior 3D visualization and better instrument maneuverability may lead to better functional outcomes compared to other methods of treatment. A couple of approaches have been developed for the treatment of thyroid gland, parathyroid glands, and neck lymph node metastases, which produce a superior cosmetic outcome. Robotic surgery can also be used to determine the primary localization of the tumor in patients with cancer of unknown primary localization. The biggest drawback of robotic surgery is the high price of equipment and its maintenance, which is why it is used only in a small number of specialized centers. Despite the drawbacks, it is a valuable treatment method which, at the moment, has its limitations. As the technology improves and more centers start to use robotic surgery for treatment of head and neck tumors, it is expected that the current drawbacks will reduce and the use of robotic surgery will extend.
