Abstract
CILJ RADA: Svrha istraživanja je proučavanjem epidemioloških, kliničkih i laboratorijskih podataka bolesnika utvrditi incidenciju i perspektivu vaskularnih bolesti kod bolesnika s dijabetes melitus tip 2 kako bi se došlo do saznanja koja bi poboljšala liječenje, ali i preveniralo komplikacije kirurških zahvata. ----- ISPITANICI I METODE: Retrospektivno istraživanje podataka iz arhive bolesnika Kliničkog bolničkog centra „Sestre milosrdnice“ koji su se liječili na Zavodu za vaskularnu kirurgiju. U obzir su uzeti bolesnici oba spola, bez obzira na dob, kojima je pridružena dijagnoza dijabetesa melitusa tip 2. ----- REZULTATI: Ukupne vaskularne promjene koje su pronađene u promatranoj skupini bile su dominantno: ateroskleroza arterija udova s gangrenom (36,2%), dijabetičko stopalo (16,7%) i okluzija i stenoza ekstraortalnih arterija kod 22 bolesnika (15,9%). Najčešći kirurški zahvati bili su nekrektomija (34,8%) i amputacija nožnih prstiju (20%). Od ukupnog broja amputacija (60 primarnih i 15 sekundarnih) izvedeno je najviše amputacija prstiju noge (41), 17 amputacija potkoljenice, 12 amputacija stopala i 11 natkoljeničnih amputacija. ----- ZAKLJUČAK: Najviše bolesnika hospitalizirano je s dijagnozom periferne arterijske bolesti kod kojih je bolest već toliko uznapredovala da rekonstrukcijski zahvat nije bio moguć. Kada se u obzir uzmu vaskularne komplikacije DM2 i potreba za amputacijom, vidljivo je da najčešća potreba za primarnom amputacijom korelira s incidencijom dijabetičkog stopala.
Abstract (english)
AIM: The purpose of the study is to determine the incidence and perspective of vascular diseases in patients with type 2 diabetes mellitus in the study of epidemiological, clinical and laboratory data to provide information that would improve treatment and prevent complications of surgical procedures. ----- METHODS: Retrospective study of archive data of patients of the Clinical Hospital Center "Sestre milosrdnice" treated at the Department of Vascular Surgery. Patients of both sexes, regardless of age, who are associated with diagnosis of type 2 diabetes mellitus were considered. ----- RESULTS: Total vascular changes found in the observed group were dominant: atherosclerosis of the gangrene leg arteries (36.2%), diabetic foot (16.7%) and occlusion and stenosis of extraortal arteries in 22 patients (15.9%). The most common surgical procedures were necrotomy (34.8%) and amputation of the toes (20%). Of the total number of amputations (60 primary and 15 secondary), toe amputations (41), 17 amputations of the lower leg, 12 amputations of the foot, and 11 intermittent amputations were performed. ----- CONCLUSION: Most patients were hospitalized with a diagnosis of peripheral arterial disease in which the disease had been so advanced that reconstruction was not possible. The most common need for primary amputation correlates with the incidence of diabetic foot.
