Abstract
Hiperoksalurija je jedan od glavnih rizičnih čimbenika za rekurentnu urolitijazu i progresivnu nefrokalcinozu. Odgovorna je za 2-20% metaboličkih rizičnih faktora identificiranih u djece i adolescenata s urolitijazom. Ovaj rad je pregled mogućih uzroka za nalaz hiperoksalurije u djeteta s urolitijazom. Tri su oblika hiperoksalurije: primarna, sekundarna i idiopatska. Umjerena idiopatska hiperoksalurija je najčešći oblik hiperoksalurije. Etiologija ove hiperoksalurije je nepoznata, ali varijabilno se pripisuje metaboličkim varijacijama koje rezultiraju prevelikim stvaranjem oksalata ili njegovom prevelikom crijevnom apsorpcijom. Primarne hiperoksalurije su autosomno recesivno nasljedne, prirođene greške metabolizma. Do danas su tri hereditarna enzimatska defekta povezana s primarnom hiperoksalurijom. Zbog veličine hiperoksalurije i njene prisutnosti od rođenja, primarne hiperoksalurije su karakterizirane posebno agresivnim stvaranjem kamenaca i zatajenjem bubrega. Sekundarna hiperoksalurija može se pojaviti kao rezultat prevelikog prehrambenog unosa ili otrovanja s prekursorima oksalata (prehrambena hiperoksalurija) ili kao rezultat crijevne hiperapsorpcije oksalata (enterička hiperoksalurija). Terapija pacijenata s hiperoksalurijom trebala bi se usmjeriti na terapiju temeljnog uzroka hiperoksalurije. Klasična terapija pacijenata s hiperoksalurijom je slična terapiji drugih stanja s kalcij oksalatnim kamencima, što uključuje dobru hidraciju i primjenu inhibitora kristalizacije kalcijevog oksalata. Veoma je važno postaviti ranu dijagnozu pacijentima s primarnom hiperoksalurijom, prije nego što se razvije sistemna oksaloza. Za veliki dio takvih pacijenata terapija izbora je kombinirana transplantacija jetre i bubrega.
Abstract (english)
Hyperoxaluria is one of the main risk factors for recurrent urolithiasis and progressive nephrocalcinosis. It accounts for 2-20% of metabolic factors identified in children and adolescents with urolithiasis. This paper is an overview of possible causes of finding hyperoxaluria in a child with urolithiasis. There are three types of hyperoxaluria: primary, secondary and idiopathic. Mild idiopathic hyperoxaluria is the most common type of hyperoxaluria. The etiology of the hyperoxaluria is unknown, but has variably been ascribed to metabolic variations resulting in increased oxalate production or to enhanced gastrointestinal absorption. The primary hyperoxalurias are autosomal recessive inborn errors of metabolism. To date, three distinct hereditary enzymatic deficiencies have been linked to primary hyperoxaluria. Due to the degree of hyperoxaluria and it´s presence from birth, primary hyperoxaluria is characterized by particularly aggressive stone formation and renal failure. Secondary hyperoxaluria may occur as a result of excess dietary intake or poisoning with oxalate precursors (dietary hyperoxaluria) or may be the result of intestinal hyperabsorption of oxalate (enteric hyperoxaluria). The therapy of hyperoxaluric patients should be directed by the underlying causes. The classic treatments for hyperoxaluric patients are similar to those of other calcium oxalate kidney stone diseases, such as good hydration and the use of calcium oxalate crystalization inhibitors. It is very important to make an early diagnosis for patients with primary hyperoxaluria, before devolepment of systemic oxalosis occurs. For a great number of them treatment of choice is combined liver and kidney transplantation.
