Abstract Uvod: Za normalan razvoj i funkcioniranje posteljice nužni su čimbenici rasta poput vaskularnog endotelnog čimbenik rasta A (VEGF-A od eng. Vascular endothelial growth factor A) koji sudjeluje u stvaranju krvnih žila posteljice procesom angiogeneze i vaskulogeneze. Insuficijentan razvoj posteljice uzrok je intrauterinog zastoja u rastu (IUGR od eng. Intrauterine growth restriction) fetusa. Nadalje, poznato je da postoje spolne razlike u funkciji posteljice koje se očituju na razini gena i kasnijoj prilagodbi fetusa na stres. Pretpostavka ovog istraživanja je da postoje i spolne razlike u izraženosti VEGF-A u posteljicama iz normalnih trudnoća i trudnoća kompliciranih IUGR-om. Ciljevi su istraživanja utvrditi postoji li razlika u izraženosti posteljičnog VEGF-A između normalnih trudnoća i trudnoća sa zastojem u rastu, te je li se te razlike opažaju ovisno o spolu djeteta.
----- Materijali i metode U istraživanje su bile uključene 32 trudnice, 16 iz normalnih (kontrola) i 16 iz trudnoća s IUGR-om. U svakoj skupini trudnice su nosile po 8 muške i 8 ženske djece. Mjereni parametri bili su gestacijska dob, porođajna masa, masa posteljice, fetoplacentarni omjer (FPR od eng. Feto-placental ratio) i imunohistokemijska izraženost VEGF-A koja je mjerena semikvantitativno.
----- Rezultati: Između kontrolne i IUGR skupine statistički značajna razlika opazila se u izraženosti VEGF-A (p=0,005) koja je povećana u IUGR skupini, porođajnoj masi (p<0,001) i masi posteljice (p<0,001). Između dvije skupine postoji statistički značajna razlika u gestacijskoj dobi, ali ona nije klinički značajna. Izraženost VEGF-A po spolu unutar kontrolne i IUGR skupine te među njima nije se statistički razlikovala (p>0,05), iako je češće povećana kod IUGR skupine, posebice u ženskim posteljicama. Statistički značajna razlika po spolu između tih skupina opazila se u porođajnoj masi (p<0,001), masi posteljice (p<0,001) i gestacijskoj dobi (p=0,001).
----- Zaključak: Izraženost posteljičnog VEGF-A razlikuje se između normalnih trudnoća i trudnoća sa zastojem u rastu. Razlika izraženosti VEGF-a nije se opazila prilikom usporedbe te dvije skupine po spolu, dok je između skupina pronađena spolna razlika u porođajnoj masi, masi posteljice i gestacijskoj dobi.
Abstract (english) Introduction: Normal development and function of placenta requires growth factors such as vascular endothelial growth factor-A (VEGF-A) which creates placental blood vessels in processes of angiogenesis and vasculogenesis. Insufficient development of placenta is cause of fetal intrauterine growth restriction (IUGR). Furthermore, it is known that there are sex differences in function of placenta in genes and fetal adaptation to stress events. The hypothesis of this study is that there are sex differences in expression of placental VEGF-A between normal pregnancies and pregnancies complicated with IUGR. Aims of this study are: to determine if there are sex differences in expression of placental VEGF-A between normal pregnancies and pregnancies complicated with IUGR, and are those differences sex related.
----- Materials and methods: This study included 32 pregnant women, 16 from normal pregnancies (control) and 16 from IUGR pregancies. In every group women were carrying 8 male and 8 female children. Measured parameters were gestation length, birth weight, placental weight, feto-placental ratio (FPR) and expression of VEGF-A, which was measured semiquantitatively.
----- Results: Between control and IUGR group there was statisticaly significant difference in expression of VEGF-A (p=0.005) which was elevated in IUGR group, birth weigth (p<0.001) and placenta weigth (p<0.001). There is statistically significant difference in gestation length between these two groups, but without clinical significance. Sex differences in expression of placental VEGF-A within each control and IUGR group and between them were not statistically significant (p>0.05), although it is more often elevated in IUGR group, especially in female placentas. When considering sex, statistically significant differences between these groups were observed in birth weigth (p<0.001), placental weigth (p<0.001) and gestation length (p=0.001).
----- Conclusion: There are differences in expression of placental VEGF-A between normal pregnancies and pregnancies complicated with IUGR. Difference in expression of VEGF-A was not observed between these two groups when considering sex, but was evident in birth weight, placenta weight and gestation length.
