Abstract
Perioralni dermatitis (POD) česta je dermatoza, nerijetko kroničnog tijeka. Najčešće obolijevaju žene, u dobi od 15 do 45 godina. Osim klasičnog oblika perioralnog dermatitisa, postoji lupoidni i granulomatozni perioralni dermatitis. U dječjoj dobi češće se javlja granulomatozni oblik, koji obično nastaje u predpubertetskom razdoblju i to većinom u dječaka. U etiologiji perioralnog dermatitisa važna je prolongirana uporaba lokalnih preparata, od kojih su najvažniji lokalni kortikosteroidi. Od ostalih čimbenika za nastanak bolesti važni su: razni kožni iritansi, fizikalni čimbenici, hormonski čimbenici i drugi. Osnovni patogenetski čimbenik jest poremećaj funkcije epidermalne barijere. Klinička slika ove bolesti je karakteristična i time je temelj postavljanja dijagnoze. Radi se o papulovezikuloznoj erupciji u perioralnoj regiji, s tipičnom uskom pošteđenom zonom oko ruba usnica. Promjene mogu biti smještene i u perinazalnom i periokularnom području, a postoji mogućnost njihovog konfluiranja. Tipični simptomi su osjećaj pečenja i boli zahvaćenog područja, uz povećanu suhoću kože. Granulomatozni perioralni dermatitis dječje dobi razlikuje se prvenstveno po dobi pacijenata koje zahvaća i po nalazu biopsije kože. Terapija se sastoji od lokalne i sustavne terapije. Terapijski pristup ovisi o težini kliničke slike i dobi pacijenta.
Abstract (english)
Perioral dermatitis (POD) is common dermatosis, often with a chronic course. It primarily affects women, aged 15 to 45 years. Besides the classical form of POD, there is a lupus-like and granulomatous perioral dermatitis. Granulomatous form is more common in childhood. It affects children in prepubertal period, mostly boys. In etiology of POD prolonged use of topical products is important, of which the most important are topical corticosteroids. Other factors important for the occurence of the disease include various skin irritants, physical factors, hormonal factors and others. The main pathogenic factor is epidermal barrier dysfunction. Clinical presentation of the disease is characteristic and thus the basis of diagnosis. It presents as a papulovesicular eruption in the perioral region, with typicalnarrow sparred zone around the edge of the lips. Changes can be located in perinasal and periocular area, and there is a possibility of their confluence. Typical symptoms include burning sensation and pain of the affected areas, with increased skin dryness. Granulomatous perioral dermatitis in children differs primarily by the age of patients that are affected and histologic findings. Therapy consists of topical and systemic therapy. Therapeutic approach depends on the severity of the clinical presentationand the patient's age.
