Abstract Uvod i cilj: Snižena koncentracija vitamina D i metabolički sindrom (MetS), uključujući njegove sastavnice, dovode se pojedinačno u vezu s povećanim rizikom za razvoj karcinoma (Ca) dojke. Nekoliko studija provedenih u žena s novodijagnosticiranim Ca dojke pokazalo je da lošija prognostička obilježja Ca dojke imaju žene s deficijencijom 25-hidroksikolekalciferola (25(OH)D) i žene s MetS, promatrajući ih odvojeno. Deficijencija vitamina D je često prisutna uz MetS a koncentracije 25(OH)D u krvi inverzno ili pozitivno koreliraju s individualnim sastavnicama MetS. Namjera ovog presječnog istraživanja je usporediti kliničko-patološka obilježja u žena s novodijagnosticiranim Ca dojke koje imaju i deficijenciju 25(OH)D i MetS, naspram žena koje imaju jedno ili niti jedno od ovih stanja. -----
Metode: Podaci za MetS (prema IDF kriterijima) i deficijenciju 25(OH)D (serumske koncentracije <50 nmol/l) i uzorci krvi prikupljeni su prije započinjanja liječenja u 203 prospektivno uzastopno uključene žene s novodijagnosticiranim operabilnim Ca dojke. Analizirani su nalazi patohistoloških obilježja tumora dojke rutinski određivanih nakon operativnog zahvata. -----
Rezultati: U ovom istraživanju u ukupnoj populaciji istodobno prisustvo deficijencije 25(OH)D i metaboličkoga sindroma nije se pokazalo udruženim s obzirom na obilježja Ca dojke promatrajući status ER, PR i HER2, Ki-67, intrinzični tip tumora, histološki gradus, TNM stadij, broj pozitivnih limfnih čvorova, prisutnost više od jednog tumora kod iste bolesnice, promjer invazivnih tumora, kao niti broj mitoza. U ukupnoj populaciji deficijencija 25(OH)D pokazala se statistički značajno češćom kod luminalnog tipa A i trostruko negativnih tumora, dok je bila značajno rjeđa kod luminalnog tipa B (P=0,027). Uz prisustvo MetS-a promjer invazivnih tumora bio je statistički značajno veći (P=0,014). Postmenopauzalne žene su statistički značajno rjeđe (P<0,001) imale više od jednog tumora u usporedbi s premenopauzalnim ženama. -----
Zaključak: Čini se da su se deficijencija 25-hidroksikolekalciferola i MetS prezentirale s drugačijim obilježjima Ca dojke i ne uz lošije kliničko-patološke prognostičke nalaze kada su prisutni istodobno. Međutim, potrebna su daljnja istraživanja kako bi se razjasnio utjecaj njihove istodobne prisutnosti na obilježja novootkrivenoga Ca dojke i eventualnu prognostičku vrijednost.
Abstract (english) Background and aims: Lower concentrations of vitamin D and metabolic syndrome (MetS), including its components, have been individually associated with increased risk for breast cancer (BC) in women. Several studies performed in women with newly diagnosed BC showed poorer prognostic characteristics in women with 25-hydroxycholecalciferol (25(OH)D) deficiency and in those with MetS, investigating them separately. Vitamin D deficiency is often present in MetS and 25(OH)D blood concentration inversely or positively correlates to individual MetS components. The aim of this cross-sectional study is to compare clinical-pathological prognostic characteristics in women with newly diagnosed BC having both 25(OH)D deficiency and MetS versus women having only one condition and none, respectfully. -----
Methods: MetS (IDF criteria) and 25(OH)D deficiency (serum concentrations <50 nmol/L) data and serum samples from 203 prospectively consecutively included women with newly diagnosed operable BC were collected before treatment initiation. Routine pathological BC tissue characteristics findings were analyzed after surgery. -----
Results: In this study, concomitant presence of 25(OH)D deficiency and MetS was not jointly associated with worse BC findings for ER, PR and HER2 status, Ki-67, intrinsic type of tumors, histological grade, TNM stage, number of positive lymph nodes, presence of more than one tumor, diameter of invasive tumors, or number of mitoses; in total, postmenopausal nor premenopausal women. In total population, 25-hydroxycholecalciferol <50 nmol/L deficiency was shown to be statistically significantly more frequent in luminal type A and triple negative types, and less frequent in luminal B tumor type (P=0.027). Diameter of invasive tumors was statistically significantly higher with metabolic syndrome present (P=0.014). Postmenopausal women had statistically significantly less frequent (P<0.001) more than one tumor comparing to premenopausal women. -----
Conclusion: It appears that 25-hydroxycholecalciferol deficiency and MetS presented with different newly BC characteristics, and with no worse clinical-pathological prognostic findings when both present. However, more researches are needed to clarify influence of their concomitant presence on newly diagnosed BC characteristics and eventual prognostic value.
