Abstract
Bol je važan zdravstveni problem svugdje u svijetu, posebno kronična bol koja doprinosi invalidnosti u velikim razmjerima. Ona je uvijek subjektivna, te je ujedno zaštitni mehanizam tijela i jedan od najčešćih razloga zbog kojega se pacijenti javljaju liječniku. Danas se bol uz temperaturu, puls, krvni tlak i frekvenciju disanja smatra petim vitalnim znakom. Razlikujemo akutnu bol, kroničnu nemalignu bol i bol koja je uzrokovana zloćudnom bolešću. Bol može bitno utjecati na kvalitetu života i zbog toga je važno otkriti uzroke nastanka boli, putove prijenosa bolnog podražaja, patofiziološka zbivanja u organizmu i načine liječenja boli. Kronična bol povećava potrošnju zdravstvene zaštite, razlog je gubitka prihoda, nesposobnosti za rad i drugih problema. Na temelju dostupne znanstvene spoznaje o uzrocima kronične boli i mehanizmima koji je održavaju, razvijene su brojne metode liječenja boli i novi terapijski pristupi. Nove spoznaje o psihološkim i socijalnim aspektima boli otvaraju mogućnost dodatnih terapijskih intervencija, naročito kod kronične boli. Unatoč tome, mnogi pacijenti još uvijek nisu u potpunosti izliječeni bez obzira na vrstu liječenja koja se primjenila. Holistički pristup, humanost u samom postupku, očuvanje kvalitete života, očuvanje funkcija i rehabilitacijskog potencijala, kontrola depresije i anksioznosti smjernice su koje bi trebale biti temelj svakom terapeutu za liječenje kronične boli.
Abstract (english)
Pain is an important public health problem worldwide, especially chronic pain which contributes to disability in a large scale. Pain is always subjective and it is a protective mechanism of the body and one of the most common reasons why patients often seek medical attention. Today, pain, temperature, pulse, blood pressure and respiratory rate are considered to be five vital signs. There is a difference between acute pain, chronic non-malignant pain and pain caused by malignant diseases. Pain can significantly affect life quality and therefore it is important to find the causes of pain onset, ways of transmission of painful stimuli, pathophysiological processes in the body and ways of treating pain. Based on available scientific knowledge about the causes and mechanisms that maintain chronic pain, numerous methods of pain treatment and new therapeutic approaches have been developed. New knowledge about the psychological and social aspects of pain allow further therapeutic interventions, particularly for chronic pain. Despite of this fact, many patients are still not fully healed regardless of the treatment type that was applied. When treating pain every therapist should take the following guidelines into consideration - a holistic approach, treating with humanity, preserving the quality of life and preserving the functions and rehabilitation potential and finally, depression and anxiety control.
