Abstract U radu se želi ispitati povezanost i međusobno djelovanje radnog okoliša medicinske sestre (opasnosti, štetnosti i napori) s kvalitetom pružene zdravstvene njege. Zbog različitih razloga u zdravstvu, kao što su štednja, nezainteresiranost poslodavca, sve češće su pojave neodgovarajućih uvjeta na radnom mjestu i radnom okolišu medicinske sestre. Cilj:Cilj rada bio je ispitati upotrebljivost procjene uvjeta i načina rada (procjene rizika) u svrhu procjene očekivanih ishoda zdravstvene njege. Ispitanici i metode: U istraživanje je bilo uključeno 60 ispitanika koji rade na radnom mjestu zdravstvenih djelatnika medicinske sestre i tehničara srednje i više stručne spreme u Specijalnoj bolnici za plućne bolesti u Zagrebu. Istraživanje se provodilo putem standardiziranog Baselskog pitnika o mogućnostima provođenja njege (Basel Extent of Rationing of Nursing Care -BERNCA Vesion 1 MS 2004)., Upitnikom se ispituju indikatori važni u radu medicinskih sestara: dekubitus, pad, zadovoljstvo pacijenta i bolničke infekcije. Uvidom u dokument „Revizija procjene opasnosti“ bolnice za 2013. godinu dobiveni su podaci o radnom okolišu. Rezultati: Rezultati pokazuju da postoji problem s higijenom ruku i nadziranjem pacijenta zbog velikih odjela koji su arhitektonski loše adaptirani. Medicinske sestre na svim radilištima moraju fiksirati pacijenta ili dati odgovarajuću terapiju za smirenje jer dezorijentiranog, dementnog, pacijenta s hiperkapnijom, intubiranog pacijenta ne mogu adekvatno nadzirati. Dokument procjene rizka radnog mjesta sadrži nepotpune podatke o radnim uvjetima i okilišu medicinske sestre. Zaključak: Neadekvatan arhitektonski položaj, neadekvatno osvjetljenje, nepostojanje zvučnog signala u bolesničkoj sobi direktno utječu na sigurnost pacijenta ali i na kvalitetu zdravstvene skrbi. Rezultati istraživanja upotrebljivosti procjene uvjeta i načina rada u svrhu procjene očekivanih ishoda zdravstvene njege u ovakvom obliku nisu primjenjivi.
Abstract (english) In this study we want to shav connection and interaction betwenn the nurses work enviroment (danger,harm and effort) with the quality of healtcare. For different reasons in healthcare, such as savings, lack of interest of the employer, we are faced with increasingly poor quality of the nurses workplace and environment. Objective: Examine the usefulness of the assessment requirements and modes (risk assessment) in order to estimate the expected outcomes of care. Methods: The study included 60 subjects who work at the place of medical professionals, nurses and technicians in middle and high qualification of employees at the Hospital for Pulmonary Diseases in Zagreb. The research was carried out by the Basel standardized questionnaire about the possibilities of conducting care (Basel Extent of rationing of Nursing Care BERNCA Version 1 MS 2004). Questionnaire examining indicators are important in the work of nurses: pressure ulcer, fall, and patient satisfaction and hospital infections. Having reviewed the document "risk assessment" for the 2013th year of the obtained data on the work environment. Results: Results show that there is a problem with hand hygiene and the monitoring of the patient because of the large departments that are architecturally badly adapted. Nurses must the bind the patient or give appropriate medication to calm it disoriented, demented, patients with hypercapnia, intubated patient cannot be adequately monitored. The document “risk assessment” workplace contains incomplete information on working conditions and impact environment nurses. Conclusion: Inadequate architectural solution, inadequate lighting, lack of a sound signal in the patient's room directly affect the safety of the patient but also the quality of health care. The research results of usability conditions and modes of operation in order to estimate the expected outcomes of health care in this form are not applicable.
