Abstract
Sindrom policističnih jajnika (PCOS) smatra se najčešćom endokrinopatijom u žena reproduktivne dobi kojemu etiologija još uvijek nije u potpunosti razjašnjena, a prevalencija mu iznosi 15-22% ako se primjenjuju Rotterdamski kriteriji za postavljanje dijagnoze. Oni obuhvaćaju poremećaje menstruacijskog ciklusa, hiperandrogenemiju te policističan izgled jajnika na ultrazvuku, a najmanje 2 od navedenih 3 moraju biti prisutna za pravilno postavljanje dijagnoze. Klinička slika PCOS-a vrlo je heterogena, a mnogi simptomi i znakovi narušavaju kvalitetu života. Uz već dobro poznate kliničke manifestacije koje se tu mogu svrstati, a to su hirzutizam, akne, poremećaji menstruacijskog ciklusa, neplodnost i pretilost, danas se sve više pažnje pridaje i psihičkom aspektu kvalitete života, a ona se najčešće procjenjuje pomoću upitnika samoprocjene (PCOSQ, MPCOSQ, SF-36). Smatra se da je kod žena s PCOS-om veća vjerojatnost pojave depresije, anksioznosti, psihoseksualne disfunkcije te poremećaja hranjenja. Depresija i anksioznost pojavljuju se u srednje teškom do teškom obliku te tako značajno umanjuju kvalitetu života žena s PCOS-om. Također, žene s PCOS-om negativno doživljavaju vlastito tijelo zbog karakterističnog fizičkog izgleda koji tome doprinosi. Na fizički izgled najviše utječe hiperandrogenemija uzrokujući hirzutizam, akne i alopeciju. Sukladno tome, liječenjem usmjerenim na hiperandrogenemiju popravlja se kvaliteta života. Osim hiperandrogenemije, cilj liječenja mogu biti inzulinska rezistencija, pretilost te oligoanovulacija.
Abstract (english)
Polycystic ovary syndrome (PCOS) is considered to be the most common endocrinopathy in women of reproductive age, its etiology is still unclear and the prevalence is 15-22% as noted by the Rotterdam criteria for diagnosis. These include menstrual irregularities, hyperandrogenemia and the polycystic appearance of the ovaries on ultrasound and for a proper diagnosis two of the three must be present. The clinical presentation of PCOS is very heterogeneous and many signs and symptoms impair quality of life. Although there are already well known clinical manifestations that can be identified such as hirsutism, acne, menstrual disorders, infertility and obesity, today it pays more and more attention to the psychological aspects of quality of life which can be assesed with questionnaires (such as PCOSQ, MPCOSQ, SF-36). Depression, anxiety, psychosexual dysfunction and eating disorders are more likely to occur in women with PCOS. Depression and anxiety occur in a moderate/severe way and thus significantly affect the quality of life in women with PCOS. Also, they have negative body image because of the characteristic physical appearance. Mostly hyperandrogenemia leads to characteristical physical appearance resulting in hirsutism, acne and alopecia. Hyperandrogenemia can be treated and that way quality of life can be improved. The target of treatment can also be insulin resistance, obesity and oligo-ovulation.
