Abstract
Alergijski kontaktni dermatitis je upala kože koja nastaje nakon kontakta s alergenom tvari koja izaziva alergijsku reakciju. Alergijski kontaktni dermatitis se manifestira neoštro ograničenim kožnim promjenama, a može se pojaviti na svim dijelovima tijela. Kožne promjene se obično pojavljuju 24-48 sati nakon kontakta s alergenom na koji je organizam prethodno senzibiliziran. Nastaje kao posljedica kasne preosjetljivosti stanično posredovana ( tip IV. reakcije prema Coombsu i Gellu). Postavljanje dijagnoze temelji se na kliničkoj slici, detaljnim prikupljanjem i analizom anamnestičkih podataka, te kožnim testiranjem. U postavljanju dijagnoze epikutano testiranje (patch test) je zlatni standard. Najčešći izvori alergena kod djece su metali (nikal, koji je vodeći uzročnik alergijskog kontaktnog dermatitisa kod djece, zatim kobalt i kalij), kemijski sastojci u kozmetičkim proizvodima i proizvodima za osobnu higijenu, obuća, privremene tetovaže i biljke. Učestalost alergijskog kontaktnog dermatitisa kod djece je u porastu, stoga rano postavljanje dijagnoze, identifikacija uzročnih alergena epikutanim testiranjem, te njegovo uklanjanje iz okoline i daljnje izbjegavanje temelj su primarne prevencije bolesti.
Abstract (english)
Allergic contact dermatitis is an inflammation of the skin that occurs after contact with an allergen of a substance that causes an allergic reaction. Allergic contact dermatitis is manifested by skin changes which are not well defined,and it can occur on all part of the body. Skin changes usually appear 24-48 hours after contact with an allergen to which the body has previously been sensitized. It occurs as a consequence of late cell-mediated hypersensitivity (type IV reaction according to Coombs and Gell). Diagnosis is based on the clinical picture, detailed collection and analysis of anamnestic data and skin testing. In making a diagnosis, patch testing is the gold standard. The most common sources of allergens in children are metals (nickel is the leading cause of allergic contact dermatitis in children, also cobalt and potassium), chemical ingredients in cosmetics and personal care products, footwear, temporary tattoos and plants. The incidence of allergic contact dermatitis in children is increasing, therefore early diagnosis, identification of causal allergens by patch testing and its removal from the environment and further avoidance are the basis of primary disease prevention.
