Abstract
Pušenje duhana i astma zasebno predstavljaju javnozdravstvene probleme i iziskuju značajne zdravstvene troškove. Proučavanje patofizioloških mehanizama kojim pušenje i astma uzrokuju patološke promjene u dišnom sustavu vrlo je zahtjevan proces, pogotovo kada se pokušaju razumjeti u kombinaciji. Novije spoznaje o upalnom mehanizmu u astmi i podjela astme na endotipove i fenotipove predstavljaju novi korak u terapijskom pristupu bolesnicima s astmom, posebno onima koji boluju od teške astme. Pušenje kao okidač i modulator upalnog procesa utječe na endotip i fenotip astme te na biljege teške astme. Biljezi teške astme predstavljaju biološke markere koji se koriste u određivanju tipa astme te kao kriterij u primjeni trenutno dostupne biološka terapija za eozinofilni ili T2 tip astme. Utjecaj pušenja na biološke markere i mogući utjecaj na terapijski odgovor predmet je novijih istraživanja uzimajući u obzir da su pušači uglavnom bili isključivani iz ranijih istraživanja o astmi.
Abstract (english)
Smoking tobacco and asthma represent public health problems and carry a significant economic burden in healthcare. Studying the pathological mechanisms of smoking and asthma responsible for pathological changes in the respiratory system is challenging, especially when they are combined. Recent studies about the inflammatory mechanisms in asthma and classifying asthma into phenotypes and endotypes represent a new step in the therapeutic approach to asthma patients, especially those suffering from severe asthma. Smoking, as the inflammatory trigger and modulator, affects asthma endotype and phenotype, as well as the biomarkers of severe asthma. Biomarkers are used to assess the type of asthma and as a criteria for the use of currently available biologics in eosinophilic or T2 asthma type. The impact of smoking on biological markers and the potential impact on therapeutic response has been the subject of recent research taking into consideration that smokers have largely been excluded from the previous research.
