Abstract
Svijet se tijekom 2020. godine našao u pandemiji koja je, gotovo preko noći, duboko promijenila sve aspekte ljudskih života, čak i u najudaljenijim dijelovima svijeta. Onkološki se bolesnici od samog početka pandemije smatraju posebno osjetljivom skupinom bolesnika, počevši od početnih izvještaja o višoj incidenciji i prevalenciji zaraze virusom, težoj kliničkoj slici i višoj smrtnosti od bolesti COVID-19 u osoba s rakom nego u općoj populaciji. S druge strane, poznata je velika opasnost koja leži čak i u kratkom odlaganju onkološkog liječenja. Tako su se onkološki bolesnici i njihovi liječnici našli u situaciji da moraju birati između Scile i Haribde, odnosno između opasnosti zaraze novim smrtonosnim virusom i dalekosežnih posljedica odgađanja liječenja i dijagnosticiranja malignih bolesti. U ovom radu prikazani su podaci iz istraživanja dostupnih u ovom trenutku; većina rezultata odnosi se na podatke iz prvog vala pandemije, odnosno iz vremena prije rasprostranjenog cijepljenja. Mnoga su istraživanja pokazala proturječne rezultate, ali ipak većina rezultata pokazuje da su bolesnici s rakom pluća i hematološkim zloćudnim bolestima pod većim rizikom zaraze sa SARS-CoV-2 virusom, kao i pod rizikom za teži oblik bolesti. Što se tiče vrste terapije kao rizičnog čimbenika, prema dostupnim istraživanjima kemoterapija predstavlja najveći rizik za zarazu i lošiji ishod bolesti COVID-19, radioterapija nosi najmanji rizik, a dosadašnja istraživanja učinka biološke terapije na tijek bolesti COVID-19 pokazuju nedosljedne rezultate.
Abstract (english)
In the year 2020, the world was suddenly engulfed in a pandemic, which has deeply affected all aspects of human life, even in the most remote parts of the globe. Cancer patients have been considered as an especially vulnerable group since the initial reports of higher incidence, prevalence, greater severity of clinical presentation and higher mortality rates when compared to the general population. On the other hand, the danger of even slight delays in cancer treatment and diagnostics is well-known. Thus, the cancer patients and their doctors were put in a position to choose between Scylla and Charybdis, or in other words to choose between the dangers of new, deadly infection and dire consequences of delays in cancer treatment and diagnostics. This thesis presents the data from studies available at the moment; most of the results are based on the data from the first wave of the pandemic, that is from the time before widespread vaccination. Many of the studies had contradictory results, but most of them indicated that patients with lung cancer and haematological malignancies are at higher risk for SARS-CoV-2 virus infection and developing more severe COVID-19. In terms of cancer treatment types as possible risk factors, available studies show that chemotherapy is associated with the highest risk of infection and severity of COVID-19, radiotherapy presents lowest risk and studies of the effect of biological therapy on COVID-19 to this day show inconsistent results.
