Abstract Spavanje je normalna, fiziološka manifestacija promijenjenog stanja svijesti i snižene motoričke aktivnosti. Zbog sve veće učestalosti, poremećaji spavanja danas su veliki medicinski i javnozdravstveni problem. Poremećaji spavanja česti su u shizofreniji, no unatoč visokoj pojavnosti ne pripadaju dijagnostičkim kriterijima shizofrenije, nego se radi o popratnim poremećajima koji se često prijavljuju u sklopu osnovne bolesti. Velika učestalost problema sa spavanjem primijećena je u populaciji bolesnika pod rizikom za razvoj psihoze te se oni mogu proučavati kao prediktorni čimbenici tranzicije u psihozu u visoko rizičnih pacijenata ili kao vjerodostojni prediktori relapsa u već dijagnosticiranih bolesnika. Djelomična ili potpuna deprivacija spavanja vodi do ozbiljnih posljedica po psihičko i fizičko zdravlje pojedinca te ima značajan utjecaj na cjelokupnu kvalitetu života. Istraživanja su identificirala dvanaest čimbenika koji imaju najveći utjecaj na spavanje u shizofrenih bolesnika, a to su: okoliš neprikladan za spavanje, nedostatak danje i večernjih aktivnosti, poremećen cirkadijani ritam, bijeg u spavanje, strah od kreveta, noćne more, noćna buđenja, spavanje prekinuto glasovima ili paranoidnim doživljajima, zabrinutost, nuspojave neuroleptika i redukcija hipnotika. Problemima spavanja u shizofreniji treba pristupiti tako da se prvo odredi njihov uzrok, odnosno svi čimbenici koji mogu imati utjecaj na kvalitetu spavanja u pojedinca te ovisno o njima prilagodi terapija. Unatoč tome što su poremećaji spavanja intrinzični dio shizofrenije i dalje ne postoje jasne smjernice koje bi stavile fokus na njihovo liječenje. Trenutačna istraživanja pokazuju veliku korist primjene kognitivno-bihevioralne terapije jer je njenom primjenom uspješno smanjen intenzitet poremećaja spavanja, uz smanjeno odustajanje pacijenata od terapije i smanjene potrebe za korištenjem hipnotika.
Abstract (english) Sleep is a normal, physiological manifestation of an altered state of consciousness and decreased motor. Due to its increasing incidence, sleep disorders are a major medical and public health problem today. Sleep disorders are common in schizophrenia, but despite their high incidence, they do not belong to the diagnostic criteria of schizophrenia but are concomitant disorders that are often reported as part of the underlying disease. A high incidence of sleep problems has been observed in the population of patients at risk for developing psychosis. They can be considered as predictors of transition to psychosis in high-risk patients or as credible predictors of relapse in already diagnosed patients. Partial or complete sleep deprivation leads to serious consequences for the mental and physical health of the individual and has a significant impact on the overall quality of life. Studies have identified twelve factors that have the greatest impact on sleep in schizophrenic patients, namely: environment unsuitable for sleep, lack of day and evening activities, disturbed circadian rhythm, sleep deprivation, fear of bed, nightmares, night awakenings, sleep interruption voices or paranoid experiences, anxiety, neuroleptic side effects, and hypnotic reduction. Sleep problems in schizophrenia should be approached by first determining their cause, or all the factors that may affect the quality of sleep in an individual, and adjusting therapy accordingly. Despite sleep disorders being a proven intrinsic part of schizophrenia, there are still no clear guidelines to focus on their treatment. Current research shows a great benefit of cognitive-behavioral therapy because its application has successfully reduced the intensity of sleep disorders, and it also significantly reduced the need to use hypnotics.
