Abstract Depresija je najčešći mentalni poremećaj u općoj populaciji. S obzirom na heterogenost i kliničku prezentaciju depresije, izazov predstavljaju dijagnostički kriteriji i specifično liječenje. Etiologija depresivnog poremećaja je izrazito složena te se pokušava objasniti brojnim biološkim te psihodinamskim i kognitivno-bihevioralnim teorijama. ... More postavljamo s pomoću kriterija MKB-10 ili DSM-5. Za točnije postavljanje dijagnoze, korisni mogu biti neurotransmiteri, te neuroendokrini, upalni i drugi markeri. U upotrebi su brojne ljestvice koje kliničari upotrebljavaju za objektivnu procjenu simptoma depresije. U liječenju depresije primjenjuju se psihofarmakoterapija, psihoterapija te alternativne metode liječenja. U posljednje vrijeme istražuje se uloga ω-3 PUFA u razvoju depresije, te njihova primjena u liječenju depresije. Prehrana siromašna ω-3 PUFA povećava rizik za razvoj simptoma depresije te je dokazana nelinearna inverzna povezanost između umjerene konzumacije ribe te unosa ω-3 PUFA i životne prevalencije depresije. Pretpostavlja se da niska razina dugolančanih ω-3 PUFA može utjecati na hormonalnu signalizaciju i na funkciju neurotransmitera, a njihov je deficit povezan i s povećanjem neuroupalnih procesa te s poremećenom neurogenezom. Uloga ω-3 PUFA u liječenju depresije temelji se na njihovim protuupalnim i neuroprotektivnim učincima. Većina istraživanja govori u prilog primjene eikozapentaenske kiseline (EPA) kao dodatne terapije u liječenju depresije, dok se izolirana primjena dokozaheksaenske kiseline (DHA) nije pokazala učinkovitom. Rezultati istraživanja djelotvornosti primjene ω-3 PUFA u žena koje boluju od perinatalne depresije, oprečni su. Unatoč očitoj zdravstvenoj dobrobiti primjene ω-3 PUFA kod osoba koje boluju od depresije, potrebno je provesti još istraživanja kako bi se ustanovila njihova uloga u razvoju depresije te njihov potencijal kao modalitet liječenja depresije. Abstract (english) Depression is the most common mental disorder in the general population. Given the heterogeneity and clinical presentation of depression, diagnostic criteria and specific treatment are challenging. The etiology of depressive disorder is extremely complex, and attempts are made to explain it by numerous biological and psychodynamic and cognitive-behavioral theories. ... More stand out. We diagnose depressive disorder using the ICD-10 or DSM-5 criteria. For a more accurate diagnosis, neurotransmitters, neuroendocrine, inflammatory, and other biomarkers may be useful. Numerous scales are used by clinicians with the aim to objectively assess symptoms of depression. Psychopharmacotherapy, psychotherapy, and alternative methods are used in the treatment of depression. Recently, the role of ω-3 PUFAs in the development of depression, and their application in the treatment of depression, have been investigated. A diet low in ω-3 PUFAs increases the risk of developing depressive symptoms, and a nonlinear inverse association between moderate fish consumption and ω-3 PUFAs intake, and the lifetime prevalence of depression, has been observed. It is hypothesized that low levels of long-chain ω-3 PUFAs may affect hormonal signaling and neurotransmitter function, and their deficiency is also associated with increased neuroinflammatory processes and impaired neurogenesis. The role of ω-3 PUFAs in the treatment of depression is based on their anti-inflammatory and neuroprotective effects. Most studies support the use of eicosapentaenoic acid (EPA) as an adjunct therapy in the treatment of depression, while the isolated use of docosahexaenoic (DHA) has not been shown as much effective. The results of a study on the effectiveness of ω-3 PUFAs in women diagnosed with perinatal depression are contradictory. Despite the obvious health benefits of using ω-3 PUFAs in patients suffering from depression, more research is needed to establish their role in the development of depression as well as their potential as a treatment modality for depression.
