Abstract Imamo najobrazovanije društvo u povijesti i puno smo toga postigli. Ipak, ponekad nam
uspjeh izmiče iz razloga koji se mogu izbjeći. Ne samo da su ti propusti česti, od medicine do
financija - već su i frustrirajući. Atul Gawande tvrdi da je razlog očit: „Opseg i složenost onoga
što znamo nadmašilo je naše individualne sposobnosti da ispravno, sigurno ili pouzdano
ostvarimo svoje prednosti. Znanje nas je i spasilo i opteretilo.” Kao odgovor na sve veću
složenost, postali smo sve specijaliziraniji. Rastuća širina i količina znanja nisu jedine koje čine
stvari kompliciranijima, iako one svakako značajno doprinose. Najveću ulogu ima provođenje
postupaka. U svakom području, od medicine do gradnje, postoji niz praktičnih postupaka,
politika i najboljih praksi. Suvremene profesije, poput medicine, sa svojim blistavim uspjesima
ali i neuspjesima predstavljaju značajan izazov: „Što kad stručnost nije dovoljna? Što kad čak
i super-stručnjaci pogriješe? ” Kako bi to prevladali, potrebna nam je strategija. Nešto što se
temelji na iskustvu i koristi znanje koje ljudi imaju, ali nekako i nadoknađuje naše neizbježne
ljudske nedostatke. Treba nam kontrolna lista.
Znanost o osiguranju kvalitete postala je integralni aspekt medicinske prakse. Jedna
kritična komponenta ovog novog razdoblja je razvoj, primjena i usavršavanje kontrolnih lista.
Kirurg i novinar Atul Gawande je napisao knjigu „The Checklist Manifesto: „Kako napraviti
stvari ispravno“ gdje navodi jednostavnu pretpostavku: „Kontrolne liste su učinkovit način za
smanjenje pogreške“. Gawande ne govori samo o kontrolnom popisu, već navodi i svoje osobno
iskustvo u osmišljavanju i provedbi kontrolne liste u sklopu projekta Svjetske zdravstvene
organizacije (SZO) za smanjenje obolijevanja i smrtnosti zbog operacija. U onim bolnicama u
kojima je provedena Kirurška sigurnosna lista SZO-e, kirurške su se komplikacije smanjile u
prosjeku za više od trećine. To je značajno smanjenje i spasilo je stotine života.
Upotreba sigurnosnih kontrolnih lista u medicini brzo se razvija. Kontrolne liste za
sigurnost pacijenata, koje su dugo vremena uporište u kirurškim operacijskim dvoranama, još
uvijek su nedosljedno dizajnirane i neujednačeno primijenjene u laboratorijima za
kateterizaciju srca.
Iako je uporaba kirurškog sigurnosnog popisa postala standardna praksa u operacijskim
dvoranama širom svijeta, upotreba kontrolnog popisa prije zahvata nije rutinski usvojena u
interventnoj kardiologiji.
Abstract (english) We have the most educated society in history and we have achieved a lot. Yet, sometimes
success eludes us for avoidable reasons. Not only are these failures common, from medicine to
finances - but they are also frustrating. Atul Gawande argues that the reason is obvious: “The
volume and complexity of what we know has exceeded our individual ability to deliver its
benefits correctly, safely, or reliably. Knowledge has both saved us and burdened us.“ In
response to increasing complexity, we have become increasingly specialized. The growing
breadth and amount of knowledge are not the only things that make things more complicated,
although they certainly contribute significantly. The biggest role is played by the
implementation of procedures. In every field, from medicine to construction, there are a number
of practical procedures, policies and best practices. Modern professions, such as medicine, with
their brilliant successes but also failures, pose a significant challenge: “What if expertise is not
enough? What if even super-experts make mistakes? ”To overcome this, we need a strategy.
Something that is based on experience and uses the knowledge that people have, but somehow
also compensates for our inevitable human shortcomings. We need a checklist. The science of
quality assurance has become an integral aspect of medical practice. One critical component of
this new period is the development, implementation and refinement of checklists. Surgeon and
journalist Atul Gawande wrote the book "The Checklist Manifesto: How to Get Things Right"
where he states a simple assumption: "Checklists are an effective way to reduce error."
Gawande not only talks about the checklist, but also cites his personal experience in designing
and implementing a checklist as part of a World Health Organization (WHO) project to reduce
morbidity and mortality due to surgery. In those hospitals where the WHO Surgical Safety List
was conducted, surgical complications were reduced by an average of more than a third. This
is a significant reduction and has saved hundreds of lives. The use of safety checklists in
medicine is evolving rapidly. Patient safety checklists, which have long been a mainstay in
surgical operating theaters, are still inconsistently designed and unevenly applied in cardiac
catheterization laboratories. Although the use of a surgical safety list has become standard
practice in operating theaters around the world, the use of a pre-procedure checklist has not
been routinely adopted in interventional cardiology.
