Abstract Svjetska zdravstvena organizacija (SZO) definira sigurnost pacijenata kao „prevenciju,
otklanjanje i unapređenje zaštite od neželjenih događaja tijekom procesa zdravstvene skrbi“.
Sigurnost bolesnika važno je javnozdravstveno pitanje i najpouzdaniji indikator kvalitete
zdravstvenog sustava. Podaci iz literature ukazuju na smanjenje razina bolesnikove sigurnosti
u vrijeme COVID-19 pandemije. Opći cilj je istražiti i usporediti razine kulture bolesnikove
sigurnosti u Klinici za infektivne bolesti dr. Fran Mihaljević za vrijeme COVID-19 pandemije.
Specifični ciljevi: Istražiti i usporediti razine kulture bolesnikove sigurnosti na odjelima Klinike
za infektivne bolesti dr. Fran Mihaljević, ispitati koliko je prijavljenih neželjenih događaja u
posljednjih 12 mjeseci i utvrditi razinu bolesnikove sigurnosti. U istraživanju je sudjelovalo
ukupno 228 ispitanika, od toga 185 medicinskih sestara/tehničara i 43 doktora medicine – stopa
odgovora 61,13%. Upitnik korišten u ovom radu jest Upitnik o kulturi bolesnikove sigurnosti
u bolnici, a njime se ispituju stavovi zdravstvenih djelatnika o bolesnikovoj sigurnosti,
pogreškama u liječenju te o izvještavanju o neželjenim događajima u bolnici. Distribucije
kvantitativnih podataka analizirane na normalnost Smirnov-Kolmogorovljevim testom
pokazale su da distribucija nije normalna. Stoga su u analizi korišteni neparametrijski analitički
postupci. Distribucije su opisane konvencionalnim mjerama deskriptivne statistike (medijan
(M) minimalna (min) i maksimalna vrijednost (max) te interkvartilni raspon (IQR). Analizirane
su Kruskall-Wallisovim testom. Distribucije kvalitativnih podataka analizirane Fisherovim
egzaktnim testom. Rezultati su interpretirani na 5%-tnoj razini značajnosti. Najveći broj
doktora medicine su radili na više različitih odjela/ ni jedan posebno u razdoblju 1-5 godina, a
medicinske sestre na Zavodu za infektivne bolesti djece ukupnog staža od 21 godinu i više.
Većina medicinskih sestara i doktora medicine radi 40-59 sati tjedno. Na razini Klinike, većina
dimenzija imale su visoku razinu, osim dimenzije Popunjenost osobljem, Najviše izmjerena
snaga je u dimenzijama Timski rad unutar odjela i Očekivanja rukovoditelja i aktivnosti koje
promiču bolesnikovu sigurnost. Nađene su statistički značajne razlike između odjela za devet
dimenzija, dok dvije, Organizacijsko učenje – kontinuirano poboljšanje i Primopredaja službe
i premještaji bolesnika unutar bolnice, nisu se razlikovale između pojedinih odjela. Visoka je
razina sigurnosti bolesnika u Klinici za infektivne bolesti dr. Fran Mihaljević. Osim
kontinuiranog truda da se visoka razina i dalje uloži, važno je zapošljavanje zdravstvenih
djelatnika kako bi se poboljšala popunjenost osobljem.
Abstract (english) The World Health Organization (WHO) defines patient safety as health care discipline
that aims to prevent and reduce risks, errors and harm which occurs to patients during provision
of health care. Patient safety is an important public health issue and the most reliable indicator
of the quality of the healthcare system. Data from the literature indicate a reduction in patient
safety levels at the time of the COVID-19 pandemic. The overall goal was to investigate and
compare patient safety culture levels at the University Hospital for Infectious Diseases dr. Fran
Mihaljević during the COVID-19 pandemic. Specific objectives: To investigate and compare
the levels of patient safety culture in the departments of the University Hospital for Infectious
Diseases dr. Fran Mihaljević, to examine the number adverse events reported in the last 12
months and to determine the level of patient safety culture. A total of 228 respondents
participated in the study, 185 of which nurses / technicians and 43 doctors of medicine – which
examines the attitudes of health professionals regarding patient safety, medical errors and
adverse eventsyielding a response rate of 61.13%. The questionnaire used in this paper is the
Hospital Survey on Patient Safety Culture (HSOPSC) reporting in the hospital. Distributions of
quantitative data analyzed for normality by the Smirnov-Kolmogorov test showed that the
distribution was not normal. Therefore, nonparametric analytical procedures were used in the
analysis. Distributions were described by conventional measures of descriptive statistics
(median (M), minimum (min) and maximum value (max) and interquartile range) (IQR),
analyzed by the Kruskall-Wallis test. The results were interpreted at a 5% level of significance.
Nurses at the Department of Infectious Diseases of children worked for 21 years and more.
Most nurses and doctors work 40-59 hours per week. At the Clinic level, most dimensions had
a high level of patient safety culture, except for the dimension Staffing, The most measured
strength is in dimensions Teamwork within units, Supervisor/manager expectations and actions
promoting safety. Statistically significant differences were found between wards for the nine
dimensions, while the two, Organizational Learning - Continuous Improvement and Handover
and Transfer of Patients Within the Hospital, did not differ between wards. There is a high level
of patient safety in the University Hospital for Infectious Diseases Dr. Fran Mihaljević. In
addition to continuous efforts to keep the high level at bay, it is important to recruit health
professionals to improve staffing.
